Abstract

A thorough knowledge of anatomy is necessary when instrumenting the upper cervical spine (UCS). Because of the frequency of anomalies of the bone and neurovascular structures in this region, multiple diagnostic studies are usually required for a comprehensive evaluation and surgical planning. The use of non-reconstructed CT is insufficient. In the majority of cases, using 3D CT reconstructions and MRI is essential. Generally, for a safe 3.5 mm screw purchase, the diameter of available bone surrounding the screw should exceed 5 mm if visual and/or fluoroscopic control is used. The specific techniques currently used in upper cervical spine reconstruction will be discussed together with their advantages and disadvantages.
