Transesophageal Tunneling Technique and Peroral Endoscopic Myotomy (POEM)

Abstract

The submucosal space was found to be an excellent area for endoscopic manipulations. This space can be used to travel with a flexible endoscope in one layer quite long distances, for example, 20 cm within the esophageal wall and/or the gastric wall. On this access peroral endoscopic myotomy (POEM) is based. The most frequently used option is an esophageal myotomy for the treatment of esophageal motility disorders. Achalasia is the most frequent disorder and has been the main indication of POEM. POEM has become also a major therapeutic option for other esophageal spastic motility disorders such as achalasia-like conditions, distal esophageal spasm, and Jackhammer esophagus.
