Abstract
This introductory chapter to the practice of pediatric surgical pathology describes a detailed recommended protocol to examine surgical biopsies or resection specimens in a systematic way, preserve and process them in a way where portions of the tissues can be used for the subsequent essential testing (if appropriate) with adjunct diagnostic procedures (immunohistochemistry and molecular analyses) that support the disease diagnosis. The principles and appropriate applications of these various contemporary diagnostic procedures are discussed. Additionally, to ensure the uniformity of pathology report formats, recommendations toward issuing a synoptic pathology report are also discussed.
The abnormal findings detected, especially in oncological tissues, can provide essential information to guide post-resection treatment with combination therapy and/or targeted therapy (conducted in academic health science institutions and advanced treatment centers). The emergence of these new DNA/RNA-based complex technologies can extend the pathology diagnostic capability to diagnose micro-residual diseases. The tumor biology information detected by these technically advanced analyses can be used as an important guide to support trials to select tumor growth inhibitory agents to reduce tumor size prior to surgery, so as to provide a better surgical resection plane (especially in small pediatric patients), or to manage further postoperative treatment of residual unresectable lesions that is limited by certain anatomical locations.
