HIV-Infected Gay Men and Adherence to HIV Antiretroviral Therapies

Understanding Prevention for HIV Positive Gay Men

Abstract

Advances in antiretroviral medications have resulted in precipitous declines in HIV-associated morbidity and mortality; however, high levels of adherence are crucial to the success of HIV therapies. This chapter reviews the current state of knowledge regarding antiretroviral therapy (ART) adherence among HIV-infected gay, bisexual, and other MSM in the US, documenting the rates of ART adherence among diverse groups of MSM, contextual issues that affect adherence, and implications for secondary prevention. Findings suggest that adherence to HIV antiretroviral medications is highly variable and may differ as a result of measurement and the broader psychosocial and contextual issues affecting the lives of gay men. These issues include access to medical care, mental health (e.g., depression), substance use, treatment beliefs, complexity of treatment regimen, and the patient-provider relationship. To improve ART adherence among HIV-infected gay men, future interventions should be culturally appropriate and designed to address the complex array of factors influencing adherence among this population.
