Zusammenfassung

Die diagnostische Laparoskopie ist heute eine wichtige Ergänzung des prätherapeutischen Stagings vor multimodalen Therapiekonzepten bei gastrointestinalen Tumoren. In der sog. erweiterten diagnostischen (chirurgischen) Laparoskopie (EDL) können alle anatomischen Regionen inspiziert werden. Mit der laparoskopischen Ultraschalldiagnostik, die heute unverzichtbarer Bestandteil der EDL ist, kann auch die dritte Dimension (parenchymatöse Organe, Lymphknoten, Retroperitoneum etc.) eingesehen werden. Der Wert der EDL als komplementäre Maßnahme zur modernen bildgebenden Diagnostik konnte für das Adenokarzinom der Speiseröhre und des Magens nachgewiesen werden. In besonderen Fällen ist auch der Einsatz beim Pankreaskarzinom und bei primären oder sekundären Läsionen der Leber sinnvoll. Die Untersuchung ist aufwändig und invasiv, sodass sie nur bei fortgeschrittenen Tumorstadien, bei denen eine multimodale Therapie in Frage kommt, indiziert ist.
