Doordrinkers: dilemma’s en definitie

Er bestaat een groep chronisch alcoholverslaafden bij wie het beloop van de ziekte ongunstig is. In de hulp en zorg aan deze groep, zeker ook in de eindfase, doet zich een aantal dillema’s voor. Moeten deze mensen onder dwang behandeld worden? Zijn ze wilsbekwaam? Wat is adequate bemoeizorg? Welke rol speelt de omgeving? Er is hierover weinig geschreven; de bestaande protocollen en richtlijnen voor behandeling zijn in hoofdzaak gericht op herstel en abstinentie en nauwelijks toepasbaar bij chronici. Dit artikel is een pleidooi voor het ontwikkelen van samenhangend beleid voor deze groep. Daartoe wordt een suggestie voor een definitie gedaan en wordt voorgesteld te spreken over ‘doordrinkers’. Helder beleid kan helpen tot verbetering te komen van de hulpverlenerspraktijk bij de subgroep chronisch alcoholverslaafden en tot nader prevalentie- en zorgonderzoek. Een benadering gebaseerd op stagering en profilering wordt hierbij aanbevolen.

Current protocols and guidelines for treating alcoholics are evidence-based and mostly directed at abstinence. However, there are alcoholics for whom treatment has failed. They continue to drink because their alcohol craving is too intense and their self‑control too little to stop the alcohol intake despite the harsh physical suffering and negative social consequences. Treatment has been refused or has had no effect. Any help from health care professionals that could interrupt their drinking behavior is rejected. This article identifies the huge dilemmas facing health care professionals when dealing with chronic alcoholics who drink themselves to death, including: Are these patients competent to decide against accepting treatment?; Do we force them to accept treatment against their will?; What is the appropriate treatment that will not lead to unacceptable suffering? We offer a precise definition of chronic alcoholics and suggest that guidelines are developed on how to provide suitable care for this group of patients in light of a staging/profiling approach.
