Summary
In 167 patients with congenital and acquired heart disease (ventricular septum defect (VSD), atrial septum defect (ASD), mitral stenosis (MS), mitral insufficiency (MI), combined mitral stenosis and insufficiency (MV) aortic stenosis (AS), aortic insufficiency (AI), combined aortic stenosis and insufficiency (AV), idiopathic hypertrophic subaortic stenosis (HOK) hemodynamic measures (arterial pressure, right and left heart pressures, cardiac output, cardiac index, stroke volume, cardiac work), left ventricular volumes (enddiastolic volume, endsystolic volume, ejection fraction, regurgitant flow) and diastolic pressure-volume relationships (on the basis of diastolic pressure-volume changes) were determined during routine right and left heart catheterization and left ventriculography.
1. Left ventricular enddiastolic volume (HOK<N<MS<AS<ASD<MI<VSD<MV<AV<AI) varied 142 ml (HOK) and 324ml (AI). Ejection fraction of the left ventricle (N>HOK>ASD>AV>MI>AI>MV>AS>VSD>MS) was 72 per cent (normal) and was lowest in MS (52 per cent). Regurgitant flow of the aortic and mitral valves varied between 38–62 per cent of total stroke volume (AI, MI, MV, AV). There existed a good correlation between ejection fraction and isovolumic indices of myocardial contractility.
2. Left ventricular enddiastolic pressure as well as diastolic pressure difference (N<MV<MS<ASD<VSD<AV<HOK<AS<MI<AI) were considerably increased in the group with volume overload. Both variables (left ventricular enddiastolic pressure, diastolic pressure difference) were closely correlated. Late diastolic volume into the left ventricle was decreased in HOK and AS in comparison to the normal, it was essentially normal in ASD, VSD, MS and MV and was markedly increased in MI and AI(HOK<AS<ASD<N<MS<VSD<MV<AV<MI<AI).
3. The ratio of diastolic pressure difference and late diastolic volume inflow (dP/dV) (AI<N=MI<MV<MS<AV<VSD<ASD<AS<HOK) showed to be normal in MI and AI; it was moderately increased in MV and MS; in AV and VSD increases by 46 per cent and 52 per cent respectively were found. The greatest increase indP/dV, that is the greatest ventricular wall stiffness, was found for AS and HOK with increases by 160 per cent and 245 per cent respectively. Diastolic pressure volume relationship exhibited differences in steepness in accord with changes in diastolic compliance.
The results demonstrate changes in left ventricular compliance determined on the basis of directly measured left ventricular diastolic pressure and volume changes in congenital and acquired heart disease associated with pressure and/or volume overload. The possibilities and limitations of calculating left ventricular compliance from diastolic pressure volume relationships, the underlying mechanisms and the hemodynamic consequences are discussed.

An 167 Patienten mit angeborenen und erworbenen Herzfehlern (Ventrikelseptumdefekt (VSD), Vorhofseptumdefekt (ASD), hypertrophische obstruktive Kardiomyopathie (HOK), Mitralstenose (MS), Mitralinsuffizienz (MI), kombinierte Mitralvitien (MV), Aortenstenose (AS), Aorteninsuffizienz (AI), kombinierte Aortenvitien (AV)) wurden hämodynamische Größen (Drucke im rechten und linken Herzen, Herzminutenvolumen, Herzindex, Schlagvolumen, Herzarbeit u.a.), linksventrikuläre Volumina (enddiastolisches Volumen, endsystolisches Volumen, Auswurffraktion, Regurgitationsvolumen) sowie diastolische Druck-Volumen-Beziehungen auf der Basis diastolischer Druckänderungen und ventriculographisch gemessener diastolischer Volumenänderungen ermittelt.
1. Das enddiastolische Volumen im linken Ventrikel (HOK<N<MS<AS<ASD<MI<VSD<MV<AV<AI) variierte zwischen 142 ml (HOK) und 324 ml (AI). Die Auswurffraktion des linken Ventrikels (N>HOK>ASD>AV>MI>AI>MS>MV>AS>VSD) betrug bei dem Normalkollektiv im Mittel 72% und war bei dem Kollektiv mit MS (im Mittel 52%) am niedrigsten. Die Auswurffraktion des linken Ventrikels zeigte eine gute Korrelation zu der aus isovolumetrischen Inotropieindices ermittelten linksventriculären Inotropie. Die Regurgitationsvolumina an der Mitral- und Aortenklappe variierten zwischen 38 – 62% des totalen Schlagvolumens (AI, MI, MV, AV).
2. Der enddiastolische Druck im linken Ventrikel sowie die diastolische Druckdifferenz (N<MV<MS<ASD<VSD<AV<HOK<AS<MI<AI) war bei den druck- und volumenbelasteten Vitien erheblich gesteigert. Beide Größen wiesen eine enge Korrelation zueinander auf. Der diastolische Volumeneinstrom parallel zum diastolischen Druckanstieg war bei HOK und AS im Vergleich zur Norm verringert, bei ASD, MS und MV weitgehend im Normbereich und bei MI und AI erheblich erhöht (HOK<AS<ASD<N<MS<VSD<MV<AV<MI<AI).
3. Der Quotient aus diastolischer Druckdifferenz und spätdiastolischem Volumeneinstrom (dP/dV) (AI<N=MI<MV<MS<AV<VSD<ASD<AS<HOK) als Maß der linksventriculären Steifigkeit bzw. reziproken Dehnbarkeit war bei AI und MI im Normbereich, bei MV und MS leicht erhöht; bei AV und VSD waren Zunahmen um 46% bzw. 52% der Norm nachweisbar. Die höchsten Dehnbarkeitsabnahmen wurden für die Kollektive mit AS und HOK mit Zunahmen vondP/dV um 160% bzw. 245% der Norm ermittelt. Die diastolischen Druck-Volumen-Beziehungen wiesen entsprechend den Dehnbarkeitsabnahmen höhere Steilheiten auf.
Die Befunde zeigen eine Änderung der auf der Basis von Messungen der diastolischen Drucke und Volumina des linken Ventrikels ermittelten ventriculären Dehnbarkeit bei angeborenen und erworbenen Herzfehlern mit Druck- und Volumenbelastung. Die Möglichkeiten und Limitationen einer Beurteilung der ventriculären Dehnbarkeit aus der diastolischen Druck-Volumen-Beziehung, die zugrundeliegenden Mechanismen sowie die hämodynamischen Auswirkungen und Korrelationen werden diskutiert.
