Abstract

The evaluation of preoperative cardiac risk in patients undergoing noncardiac surgery has been a challenging and important topic over the past 25 years. Given the growing prevalence of both coronary artery disease (CAD) and noncardiac surgical procedures in the United States,1 the preoperative evaluation for cardiac risk becomes more important. The medical parameters and clinical factors first reported by Goldman et al.2 have evolved into a more systematic approach and recently into American College of Cardiology/American Heart Association guidelines.3 Although controlled prospective or randomized clinical trials are lacking in this clinical area, there are many retrospective reports and a few metaanalyses that demonstrate the utility of nuclear cardiology testing in this evaluation process.
