Zusammenfassung
Hintergrund
Es wird die Entwicklung eines bereichsspezifischen Instruments zur Messung des Kohärenzgefühls im Krankenhaus dargestellt („hospital sense of coherence“, H-SOC).
Methoden
In einem Querschnittsdesign wurden 300 Patienten eines Landeskrankenhauses befragt. Neben dem H-SOC wurden folgende Konstrukte erhoben: das allgemeine Kohärenzgefühl (SOC-L9), die aktuelle Beanspruchung (KAB), die primäre Bewertung des Krankenhausaufenthalts, die Vorhersehbarkeit der Einlieferung, die subjektive Gesundheit, die Dauer des Krankenhausaufenthalts sowie soziodemografische Daten.
Ergebnisse
Mittels konfirmatorischer Faktorenanalyse konnte hinsichtlich des H-SOC ein zufriedenstellender Fit ermittelt werden; die 3-faktorielle Lösung geht konform mit der Theorie der Salutogenese. Die Reliabilität der Skala ist zufriedenstellend (Cronbachs α = 0,794), die Validität konnte über theoriekonforme Beziehungen mit relevanten Konstrukten bestätigt werden.
Schlussfolgerungen
Die Messung des settingspezifischen Kohärenzgefühls kann dazu dienen, vulnerable Patienten zu identifizieren und die Lebenswelt Krankenhaus entsprechend zu modifizieren, sodass ein Gefühl von Verstehbarkeit, Bewältigbarkeit und Sinnhaftigkeit entstehen kann.
Abstract
Background
The development of a scale for measuring the sense of coherence specifically for the setting “hospital” is presented (H-SOC).
Methods
Using a cross-sectional design 300 patients of a regional hospital were surveyed. In addition to the H-SOC the following constructs were measured: general sense of coherence (SOC-L9), emotional stress (KAB), primary appraisal of the hospital stay, predictability of admission, subjective health, length of hospital stay, and sociodemographic data.
Results
Using confirmatory factor analysis a satisfactory fit could be revealed regarding the H-SOC. The reliability of the scale is satisfactory (Cronbach’s alpha = 0.794), and validity was proven through relations of the scale with relevant constructs that conform to the theory.
Conclusion
Measuring the setting-specific sense of coherence can help to identify vulnerable patients and to modify the hospital setting in order to promote a sense of comprehensibility, manageability, and meaningfulness.
