Conclusion. Retropubic sling technique is unlike most other pelvic reconstructive surgical procedures, as it involves blind instrument passage in a complex, three-dimensional anatomical space. The complications are acceptably low, but the surgeons must be properly trained, and maintain a good surgical volume to remain competent.
Complications can be minimized by adhering to fundamental surgical practices during the sling insertion. These include appropriate knowledge of pelvic and vaginal anatomy, careful vaginal dissection and hemostasis, diligence during the passage of the sling trocars to avoid injury or perforation to the bladder, urethra, vaginal tissue, or groin structures. Appropriate tensioning during sling deployment, and prudence for identifying intraoperative complications.
