Zusammenfassung
Durch eine massive bakterielle Überwucherung bzw. durch eine Besiedelung des Dünndarms mit eigentlich harmlosen Dickdarmbakterien (bakterielle Fehlbesiedelung) kommt es zu einer gravierenden Störung der Homöostase im bakteriellen Ökosystem (Dysbiose) mit ernsthaften Verdauungsstörungen. Das klinische Erscheinungsbild hängt entscheidend von der zugrunde liegenden Erkrankung ab. Zu den typischen Symptomen einer bakteriellen Überwucherung des Dünndarms gehören Meteorismus, Flatulenz, abdominelles Spannungsgefühl, Diarrhoe und Bauchschmerzen. Grundlage der Diagnostik ist eine gründliche Anamnese sowie Blutbild und Bestimmung von Gesamteiweiß, Albumin und Vitamin B12. Daneben können H2-Atemtest und bildgebende Verfahren zum Einsatz kommen. Rein klinisch lassen sich Reizdarmsyndrom und bakterielle Fehlbesiedelung des Dünndarms nicht auseinanderhalten. Die konservative Therapie stützt sich in erster Linie auf den Einsatz von Antibiotika. Ein anderer Therapieansatz ist die Verordnung von Probiotika, ggf. auch in Kombination mit Antibiotika.
