Zusammenfassung
Epiphysäre Frakturen der proximalen Tibia beim Kind stellen in jedem Fall schwere Kniegelenksverletzungen dar. Je nach Unfallmechanismus und Alter des Kindes können neben den fugenkreuzenden Epiphysenfrakturen auch Ausrissfrakturen der Eminentia intercondylaris und Abrissfrakturen der Tuberositas tibiae beobachtet werden. Die „typischen“ Epiphysenfrakturen (Salter-Harris III und IV) entstehen durch direkte oder indirekte Gewalteinwirkung und müssen als Gelenkfrakturen bereits bei geringer Dehiszenz operativ versorgt werden. Der Entstehungsmechanismus der Ausrissfrakturen der Eminentia intercondylaris entspricht jenem der Ruptur des vorderen Kreuzbands beim Erwachsenen. Dementsprechend ist neben der Röntgendiagnostik zusätzlich eine MRT durchzuführen, um etwaige Begleitverletzungen zu erkennen und behandeln zu können. Während undislozierte Frakturen ohne Begleitverletzungen konservativ behandelt werden können, sind dislozierte Frakturen (ab Grad II nach Meyers und McKeever) sowie bei bestehenden Begleitverletzungen operativ zu behandeln. Derzeit stellen die Reposition und transossäre Auszugsnaht den Goldstandard dar. Durch die Verwendung von resorbierbarem Nahtmaterial kann ein Zweiteingriff zur Entfernung vermieden und die Gefahr eines frühzeitigen Fugenschlusses verhindert werden. Ausrisse der Tuberositas tibiae sind verhältnismäßig selten und kommen fast ausschließlich zwischen dem 11. und 17. Lebensjahr vor, wobei das Durchschnittsalter 15 Jahre ist und in mehr als 90 % Buben betroffen sind. Bei nahezu undislozierten Frakturen (Typ I A, III A) kann eine konservative Behandlung im Oberschenkelgips bei gestrecktem Knie erfolgen, während alle dislozierten Frakturen operativ behandelt werden. Eine relative Indikation besteht bei nach intraartikulär auslaufenden Frakturen, die vor allem in der Adoleszenz prophylaktisch verschraubt werden (Typ I B, II A,B und III A,B). Bei Typ-IV-Verletzungen mit Beteiligung der dorsalen Metaphyse kann eine Plattenosteosynthese erforderlich sein.
