Zusammenfassung
Störungen der Lungenfunktion können bei Früh- und Neugeborenen zu schwerwiegenden Komplikationen in der postpartalen Anpassungsphase führen und die Prognose der betroffenen Kinder nachteilig beeinflussen. Das Atemnotsyndrom ist eine typische Krankheit Frühgeborener. Es wird durch den Mangel eines pulmonalen oberflächenaktiven Systems, des Surfactant (surface active agent) verursacht. Insbesondere die Surfactantbehandlung des Atemnotsyndroms stellt einen Meilenstein in der Entwicklung der neonatalen Intensivmedizin dar. Die bronchopulmonale Dysplasie ist eine bedrohliche, aber potenziell reversible Lungenkrankheit Frühgeborener, die meist wegen eines Atemnotsyndroms maschinell beatmet und mit erhöhten inspiratorischen Sauerstoffkonzentrationen behandelt wurden. Eine Mekoniumaspiration führt zu einer akuten Atemnotsymptomatik, die überwiegend bei übertragenen oder reifen hypotrophen Neugeborenen auftritt und durch einen kompatiblen radiologischen Lungenbefund charakterisiert ist. Die persistierende pulmonale Hypertonie (Synonym: persistierende fetale Zirkulation des Neugeborenen) ist ein lebensbedrohliches Krankheitsbild, das ebenfalls überwiegend bei reifen und übertragenen Neugeborenen auftritt und durch einen persistierenden erhöhten pulmonalen Gefäßdruck mit konsekutivem Rechts-Links-Shunt über das offene Foramen ovale den persistierenden Ductus arteriosus und intrapulmonale Shunts ohne Hinweise auf ein Vitium cordis charakterisiert ist. Weitere relevante Störungen der Neonatalperiode sind Pneumothorax, Lungenblutungen, Lungenhypoplasie, lobäres Emphysem, Zwerchfellhernien, Chylothorax und die neonatale Pneumonie.
