Zusammenfassung
Die normale Hämglobinkonzentration (Hb) zeigt im Verlauf der Entwicklung erhebliche Unterschiede, sodass die Anämie in verschiedenen Altersklassen durch variable untere Hb definiert ist. Pathogenetisch kann einer Anämie eine Bildungsstörung, eine verkürzte Lebensdauer der Erythrozyten bzw. eine ineffektive Erythropoese mit Untergang der erythropoetischen Vorläuferzellen bereits im Knochenmark oder ein Blutverlust zugrunde liegen. Differenzialdiagnostisch ist nach sorgfältiger Erhebung der Anamnese und des körperlichen Untersuchungsbefundes die Einteilung nach der Größe der Erythrozyten und nach Retikulozytenzahl hilfreich. Die Sicherung der Diagnose kann bei der Vielzahl erworbener und hereditärer, oft seltener Erkrankungen eine Herausforderung darstellen, ist bei der Mehrzahl der Patienten bei Einsatz des im Kapitel beschriebenen Algorithmus jedoch recht einfach. Die Therapie hängt von der korrekten Diagnose ab und zeigt ein weites Spektrum vom aufmerksamen Zuwarten, über die Ernährungsumstellung und Supplementation von Eisen oder Vitaminen, die Erythrozytentransfusion bis zur allogenen Stammzelltransplantation.
