Zusammenfassung

Ischämieschmerzen spielen in der Schmerztherapie eine oft unterschätzte Rolle. Ein Grund mag sein, dass Ischämieschmerzen vorwiegend in spezialisierten Fachdisziplinen wie Angiologie, Gefäßchirurgie und der interventionellen Radiologie behandelt werden. Patienten suchen „nur“ wegen der Ischämieschmerzen keinen Schmerztherapeuten auf. Andererseits ist die Prävalenz der peripheren arteriellen Durchblutungsstörung (PAVK) zumindest im höheren Lebensalter derart hoch (Diehm et al. 2004a), dass praktisch jeder Schmerztherapeut mit dieser Erkrankung irgendwann konfrontiert werden wird. Die Bedeutung der PAVK liegt darin, dass sie oft die ersten Symptome einer generalisierten Arteriosklerose präsentiert, welche die weitere Prognose der Patienten wesentlich bestimmt. Rund 60 % der Patienten mit PAVK sterben am Myokardinfarkt (Smith et al. 1996).
