Zusammenfassung

Der implantierbare automatische Kardioverter-Defibrillator (ICD) wurde 1980 erstmals bei einem Menschen in Baltimore/USA implantiert. In Deutschland wurden die ersten ICD-Implantationen im Januar 1984 in Düsseldorf und Hannover durchgeführt. Der plötzliche Herztod (PHT) gehört zu den häufigsten Todesursachen in der westlichen Welt, und jährlich erliegen in Deutschland etwa 70.000 bis 100.000 Menschen einem PHT, in den USA versterben etwa 450.000 Menschen plötzlich und unerwartet. Auch 34 Jahre nach der Erstimplantation in den USA und 30 Jahre nach den ersten Implantationen in Deutschland ist der ICD ist die beste Therapie der Wahl zur Primär- und Sekundärprophylaxe zur Verhinderung eines PHT und ist allen anderen therapeutischen Konzepten eindeutig überlegen. Die kardiale Resynchronisationstherapie (CRT) ist eine wesentliche Erweiterung der ICD-Therapie und führt nicht nur zu einer Herzinsuffizienz-, sondern auch zu einer Prognoseverbesserung.

Abstract

An automatic cardioverter defibrillator (ICD) was implanted for the first time in a human in Baltimore USA in 1980. The first ICD implants were performed in Germany in January 1984 in Düsseldorf and Hannover. Sudden cardiac death (SCD) is one of the major problems in the western world with approximately 70,000–100,000 SCD patients in Germany and 450,000 in the USA. Currently, 34 years after the first implant in USA and 30 years after the first implants in Germany, the ICD is the method of choice for primary or secondary prevention of SCD and is superior to all other therapeutic approaches. Cardiac resynchronization therapy (CRT) is an important additional therapeutic tool and leads to improvements in congestive heart failure and overall survival.
