Zusammenfassung
Der plötzliche Herztod (PHT) gehört zu den häufigsten Todesursachen in der westlichen Welt, und jährlich erliegen in Deutschland etwa 70.000–100.000 Menschen einem PHT, in den USA versterben etwa 450.000 Menschen plötzlich und unerwartet. Der PHT ist nicht durch einzelne Faktoren bedingt, sondern ist als multifaktorielles Geschehen zu verstehen. Der PHT wird in etwa 90% durch ventrikuläre Tachyarrhythmien hervorgerufen, während bradykarde Rhythmusstörungen nur in etwa 5–10% zum PHT führen. In etwa 50% der Fälle ist der PHT die Erstmanifestation einer kardialen Erkrankung. Es ist inzwischen unbestritten, dass die Frühdefibrillation zu einer wesentlichen Verbesserung der Überlebensrate von Patienten mit Kammerflimmern führt. Während für die Langzeittherapie medikamentöse Therapiestrategien nicht zu einer signifikanten Reduktion des PHT führen, ist die Implantation automatischer Defibrillatoren als Primär- und Sekundärprävention etabliert.
Abstract
Sudden cardiac death (SCD) is one of the major problems in the western world, with approximately 70,000–100,000 SCD victims in Germany and 450,000 in the US. SCD is a multifactorial problem. It is caused by ventricular tachyarrhythmias in approximately 90% of patients, and by bradyarrhythmias in 5%–10%. In 50% of victims, SCD is the first manifestation of heart disease. There is general agreement that early defibrillation is an effective tool to treat patients with ventricular fibrillation. As a long-term strategy, antiarrhythmic drug therapy is not successful in preventing SCD, whereas the implantable cardioverter defibrillator is the method of choice for primary or secondary prevention.
