Zusammenfassung
Einleitung
Obwohl mehr als die Hälfte der geriatrischen Klinikpatienten Tagesschläfrigkeit zeigen, werden Schlafstörungen oder Tagesschläfrigkeit nicht regelhaft im Rahmen eines geriatrischen Assessments erfasst. Die Epworth Sleepiness Scale (ESS) ist das am häufigsten verwendete Instrument zur Erfassung von Tagesschläfrigkeit. Die ESS wurde auch für alte Menschen validiert, nicht jedoch für geriatrische Patienten.
Methodik und Ergebnisse
Wir analysierten daher die Ergebnisse der ESS bei geriatrischen Patienten. Fünfundneunzig geriatrische Klinikpatienten mit einem medianen Alter von 84 (Interquartilsabstand, IQR: 81–88) Jahren füllten die ESS komplett aus. Der Median des Summenscores sowie der Interquartilsabstand (IQR) der ESS lagen bei 4 (2–5), bei den Männern bei 4 (3–5) und bei den Frauen bei 4 (2–6). Die Korrelationen der einzelnen Items mit dem Gesamtscore waren schwach, die Werte für Cronbachs Alpha lagen zwischen 0,37 und 0,55. Eine Faktorenanalyse der Items zeigte, dass sich 2 Faktoren abgrenzen lassen. Ein Faktor lädt die Items 1 (0,743), 2 (0,777) und 7 (0,755), ein zweiter Faktor lädt die Items 5 (0,696) und 6 (−0,707). Bei Einschluss der 3 Items mit der höchsten Ladung (Items 1, 2 und 7) errechnet sich ein Cronbachs Alpha von 0,673.
Schlussfolgerung
Die aktuelle deutsche Version der ESS erreicht nicht die Gütekriterien, die für eine Anwendung in der Geriatrie erforderlich sind. Hier sind Weiterentwicklungen oder Adaptationen der ESS erforderlich.
Abstract
Introduction
Although more than half of geriatric in-hospital patients show daytime sleepiness, sleep disorders or daytime sleepiness are not regularly recorded in a geriatric assessment. The Epworth Sleepiness Scale (ESS) is the most commonly used tool for monitoring daytime sleepiness. The ESS has also been validated for the elderly, but not for geriatric patients.
Methods and results
We therefore analyzed the results of the ESS in geriatric patients. Ninety-five patients with a median age of 84 and an inter quartile range (IQR) of 81–88 years completed the ESS. The median total score and the IQR of the ESS were 4 (IQR: 2–5), 4 (IQR: 3–5) for the men, and 4 (IQR: 2–6) for the women. The correlations of the single items with the overall score were weak; the values for Cronbach’s alpha ranged between 0.37 and 0.55. A factor analysis of the items showed that two factors can be distinguished. One factor loads items 1 (0.743), 2 (0.777), and 7 (0.755), and a second factor loads items 5 (0.696) and 6 (−0.707). Including the three items with the highest load (items 1, 2, and 7), Cronbach’s alpha was calculated as 0.673.
Conclusion
The current German version of the ESS does not meet the quality criteria required for use in geriatrics. Further developments or adaptations of the ESS are required.
