Die gutachtliche Bewertung der traumatischen Schulterinstabilität

Abstract

To answer the question, if the complaints of a patient with an instability of the shoulder are related to an accident, it is important to know something about the possible causes of an instability and the clinical, radiological and (ifavailable) intraoperative findings, i.e. the findings immediately after the accident and the findings thereafter.

The medical history of the patient before the accident, as well as the features of the accident itself also have to be explored further. Of special interest is the clinical functional examination of both shoulders during the individual medical examination. In this paper, all these characteristics are discussed and a pro-contra concept, in addition to a recommendation for classification of functional impairment, is presented.

Zusammenfassung

Die Beurteilung der Fragestellung, ob eine Beschwerdesymptomatik und Funktionsbeeinträchtigung aufgrund einer Schulterinstabilität auf einen Unfall zurückzuführen ist, erfolgt unter Berücksichtigung der möglichen Ursachen der Instabilität und unter Heranziehung unfallnaher sowie im weiteren Verlauf erhobener klinischer, radiologischer und ggf. intraoperativer Befunde, Informationen zur Vorgeschichte und zum Unfallhergang. Bedeutung beizumessen ist auch den klinischen Funktionstests zum Zeitpunkt der gutachtlichen klinischen Untersuchung. Es muss eine zusammenschauende Beurteilung unter Berücksichtigung sämtlicher Kriterien vorgenommen werden. Diese Kriterien und ein entsprechendes Pro-Contra-Konzept sowie eine Empfehlung zur Einschätzung der Funktionsbeeinträchtigung werden im Folgenden vorgestellt.
