Zusammenfassung
Neben sensibilisierenden und physikalischen exogenen Noxen stellen Chemikalien mit irritativ-toxischem Potenzial eine weitere wichtige Ursache für inhalative Lungenschädigungen dar. Expositionsmöglichkeiten sind sowohl in der Umwelt als auch arbeitsplatzbezogen möglich. Die häufigsten Umweltnoxen sind die Luftschadstoffe Tabakrauch, Stickoxide, Schwefeldioxid und Ozon. Bei der Arbeitsplatzexposition kommen ca. 230 verschiedene als chemisch-irritativ oder toxisch bewertete Chemikalien bei grenzwertüberschreitenden Konzentrationen als Auslöser infrage. Es können akute Inhalationstraumen durch einmalige unfallartig hohe Expositionen von chronischen Schädigungen durch Dauerexposition unterschieden werden. Akute Inhalationsintoxikationen können vorrangig folgende Effekte haben: akute toxische Reizung der oberen Atemwege mit Laryngitis und Tracheitis, Reizungen der Bronchien mit akuter Bronchitis, Bronchokonstriktion und ggf. Induktion eines RADS sowie Schäden im Alveolarbereich mit Lungenödem. Bei der Asphyxie werden die Erstickungsgase zwar inhalativ aufgenommen, bewirken eine Hypoämie, jedoch keine Lungenschäden im engeren Sinne. Als chronische Lungenschäden sind die chronische Bronchitis, die chronisch-obstruktive Bronchitis ggf. mit Lungenemphysem sowie das irritativ-toxische Asthma bronchiale zu nennen.
Abstract
In addition to sensitising and physical noxious matter, chemicals with toxic or irritant potential are an important factor in inhalation-induced damage of the respiratory system. Exposure may be environmental or occupational. The most common environmental air pollutants are tobacco smoke, nitrogen oxides, sulphur dioxide and ozone. Occupational exposure can arise from about 230 different chemicals evaluated as being toxic or irritant if they exceed a threshold value. Acute inhalation trauma by a single accidental high exposure should be distinguished from chronic impairment through long-term exposure. Acute intoxication primarily results in the following effects: acute toxic irritation of the upper airways, mainly the larynx and trachea, irritation of the lower airways with acute bronchitis and bronchial constriction, sometimes followed by reactive airways dysfunction syndrome (RADS) and alveolar damage with pulmonary edema. Asphyxia is induced by inhaled gases and leads to hypoxia but not to lung damage itself. Chronic impairment of the lung can cause chronic bronchitis, chronic obstructive lung disease, sometimes combined with emphysema, and toxic or irritant-induced bronchial asthma.
