Zusammenfassung
Apophysen und Epiphysen sowie deren Fugen haben in der Ontogenese den gleichen Ursprung und einen zunächst identischen Aufbau. Die später vorhandenen, deutlichen Unterschiede werden durch funktionelle Einflüsse bestimmt.
Apophysen übertragen die Zugkräfte der hier ansetzenden Sehnen mittels Sharpey-Fasern auf die Diaphyse, überbrücken die Knorpelfuge und strahlen weiter in das kollagene Fasersystem des Knochens ein. Dabei kommt es durch die induzierte Sehnentraktion zu einer Kompression der Knorpelfuge entsprechend dem Kräfteparallelogramm, da die ziehenden Kräfte über ein Gegenzugsystem aufgefangen und umgeleitet werden.
Die daraus resultierende These lautet, dass unter physiologischen Bedingungen nur eine deutlich vorerkrankte Apophyse ausreißen kann. Ein Aus- oder Abriss ist nur erklärbar, wenn krankhafte Prozesse bereits einen Schaden verursacht haben, der die Reißfestigkeit der Apophyse schwächt. Für einen verletzungsbedingten Ausriss einer Apophyse ist eine Kombination aus gewaltsamen Zug- und Scherkräften erforderlich mit der Folge von Begleitverletzungen.
Abstract
In ontogenesis, the apophysis as well as the epiphysis grow from the same origin and therefore first show an identical histological structure. The development of fundamental differences are triggered by functional influences.
Apophyses are the anchors for the traction forces that are transferred from the tendons via Sharpey’s fibers bridging the growth plate and inserting into the collagen fiber system of the bone. These specific anatomical structures around the apophysis transform tension forces through a complex system of tendon braces into compression forces in the plane of the apophyseal basis, the corresponding growth plate.
The thesis resulting from these findings concludes that under physiological conditions avulsions of apophysis occur only in pre-existing disorders.
An avulsion of the apophysis results from a degenerative process that has already led to damage in the complex tension and compression vectors. Traumatic injuries are caused by the combined strong shear and tension forces included a concomitant injury.
