Zusammenfassung
In den letzten Jahrzehnten konnte die Überlebenswahrscheinlichkeit nach schweren Verletzungen deutlich verbessert werden. Damit rücken funktionale Einschränkungen, Schmerzen, psychische Belastungen und soziale Beeinträchtigungen als Folgezustände nach dem Überleben vermehrt in den Fokus. Die Einrichtung von Traumanetzwerken hat die bestmögliche Versorgung von Schwerverletzten unter standardisierten Qualitätsmaßstäben zur Zielsetzung. Die Planung und Durchführung rehabilitativer Maßnahmen sollten frühzeitig zur Verbesserung von Körperfunktionen, Aktivitäten und der Partizipation sowie v. a. der beruflichen Wiedereingliederung und der Lebensqualität erfolgen. Als Leistungsträger kommen im Wesentlichen die Berufsgenossenschaften, die Deutsche Rentenversicherung und die gesetzlichen Krankenkassen in Betracht. Die Rehabilitation erfolgt im multidisziplinären Team unter Berücksichtigung eines biopsychosozialen Ansatzes. Gerade die Rehabilitation nach schweren Verletzungen bedarf spezifischer orthopädischer und unfallchirurgischer Kompetenz. Nur durch den gezielten Einsatz der Rehabilitation lassen sich neben den behindernden Folgen für den Einzelnen auch volkswirtschaftlich Kosten im Gesundheitssystem senken.
Abstract
During the past decades mortality following major trauma has decreased significantly. Functional impairments, chronic pain, psychological distress and social consequences following major trauma have thus become increasingly important. The implementation of trauma networks in Germany focuses on the best possible treatment of severely injured patients. Planning and performance of rehabilitation should start early to improve body functions, activities and participation, especially return to work and quality of life. Service providers for rehabilitation in Germany are the workers’ compensation board, the social pension fund and the statutory health insurance. Rehabilitation is performed in a multidisciplinary setting taking into consideration the biopsychosocial model. Specific orthopaedic and trauma surgery competence is needed in the rehabilitation of severely injured patients. Rehabilitation following trauma surgery can minimize the devastating consequences of major trauma for the individual patient and reduce costs in the health care system and national economy as well.
