Zusammenfassung
Knorpelschäden werden eingeteilt in Arthrose bzw. Knorpelverletzungen. Letztere gehen in Erstere über, setzen aber ein geeignetes Unfallereignis voraus. Da der Knorpel elastischer als der subchondrale Knochen ist, entsteht bei der direkten zunehmenden Krafteinwirkung auf den Knorpel ein subchondrales Knochenödem bis hin zur Knorpelfraktur. Zur Beurteilung und Differenzierung ist eine synergistische Diagnostik angebracht (MRT, Arthroskopie), da eine Methode allein meist zu wenig aussagekräftig ist. Die Röntgenologie ist v. a. zur Darstellung einer eventuellen Arthrose zum Zeitpunkt des Unfalls geeignet. Veränderungen derselben im zeitlichen Verlauf oder ihre Entstehung bei unauffälligem Primärbild sind ebenfalls wichtige Hinweise auf einen traumatischen Knorpelschaden. Mit der MRT können nicht nur Knorpel- und Weichteilverletzungen direkt visualisiert, sondern auch durch den Nachweis und den zeitlichen Verlauf des subchondralen Knochenödems wertvolle Anhaltspunkte für die Beurteilung eines akuten Knorpelschadens erhalten werden. Die Arthroskopie ist die derzeit exakteste Methode zur Beurteilung der oberflächlichen Knorpelschichten und erlaubt in den ersten 6–12 Wochen nach dem Trauma eine Differenzierung hinsichtlich akutem vs. vorbestehendem Schaden.
Abstract
Cartilaginous damage is divided into arthrosis and injury to cartilage. The latter merges into arthrosis, but arise only after a relevant accidental event. As the cartilage is more elastic than the subchondral bone, with the direct and increasing impact of force on the cartilage subchondral bone oedema develops, which can lead to cartilaginous fracture. A synergistic diagnostic procedure is needed particularly to show any arthritis present at the time of the accident, as one method alone mostly yields too little information. Changes in arthritis over the course of time or its development following unremarkable images initially are important indicators of traumatic damage to the cartilage. MR imaging allows not only direct visualisation of injuries to the cartilage and soft tissues, but also, by showing the presence and the temporal course of subchondral bone oedema, gives valuable pointers for the assessment of an acute cartilaginous injury. Arthroscopy is currently the most accurate method of assessing the superficial layers of cartilage and allows differentiation between acute and pre-existing damage in the first 6–12 weeks after a trauma is sustained.
