Zusammenfassung

Hintergrund

Soziale Grenzverletzung meint sowohl das Etablieren einer freundschaftsähnlichen Beziehung innerhalb der Therapie als auch parallel stattfindende private Kontakte zwischen TherapeutIn und PatientIn und weist Überschneidungen mit dem in der englischsprachigen Forschungsliteratur verbreiteten Begriff der „dual relationships“ auf. Soziale Grenzverletzungen stellen einen Bruch der schulenübergreifend geltenden Abstinenzregel dar. Trotz weiter Verbreitung sind Dynamiken dieser Grenzverletzungen bislang kaum systematisch untersucht.

Material und Methode

In einem naturalistischen Design wurden Verläufe sozial grenzverletzender Psychotherapien und Ablösungsmöglichkeiten von PatientInnen aus diesen Therapien erforscht. Es wurden 16 Beratungsfälle anhand von Dokumenten, die PatientInnen im Rahmen einer Beratung bei einer Anlaufstelle eingereicht hatten, qualitativ-inhaltsanalytisch untersucht.

Ergebnisse

Soziale Grenzverletzungen können sich auf unterschiedliche Weise manifestieren. PatientInnen hegen einerseits Zweifel an der grenzverletzenden Psychotherapie, während sie andererseits Motive für das Weiterführen der Therapie nennen (z. B. Gefühl, eine besondere Behandlung zu bekommen, drohende Einsamkeit). Weiter wurden Faktoren, die dieses Spannungsfeld aufbrechen, herausgearbeitet (z. B. Distanzierung durch den Therapeuten/die Therapeutin).

Schlussfolgerung

Zentral ist die Gleichzeitigkeit von anziehenden und abstoßenden Faktoren bezüglich der Weiterführung der Therapie bei sozialen Grenzverletzungen. In diesem Zusammenhang werden Ähnlichkeiten zu spezifischen Dynamiken des professionellen sexuellen Missbrauchs diskutiert.

Abstract

Background

Social boundary violations in psychotherapy include the establishment of a friendship-like relationship within therapy sessions as well as private contacts between therapists and patients outside of therapy sessions. The term social boundary violation overlaps with the concept of dual relationship, a term widely used in English research literature. Social boundary violations can be understood as a misconduct of the pantheoretical principle of abstinence. Even though being a common issue there is very little systematic research on the dynamics of these types of violations.

Material and methods

Courses of therapies marked by social boundary violations and means of detachment of affected patients were investigated in a naturalistic design. A total of 16 counselling cases were analyzed. The material consisted of documents the patients had submitted within the framework of counselling at a contact center and were evaluated using qualitative content analysis.

Results

Social boundary violations can be manifested in different ways. Patients in these therapies are caught between the two extremities of having doubts about the boundary violating psychotherapy and developing motives to continue the therapy (e.g. feeling of receiving special treatment, threat of loneliness). Furthermore, factors that liberate patients from this area of conflict were explored (e.g. distancing of the therapist).

Conclusion

The concomitance of discouraging as well as attracting factors with respect to continuing therapy in cases of social boundary violations is central to maintaining the dual relationship. In this context similarities to the specific dynamics of professional sexual misconduct are discussed.
