Zusammenfassung
Hintergrund
Die Ellenbogenluxation ist eine komplexe Verletzung, die zu unterschiedlichsten Verletzungsformen und -schweren führen kann. Das Therapiekonzept kann, abhängig von der Ausbildung und der Erfahrung des behandelnden Arztes, sehr unterschiedlich sein. Bis heute stellt die Luxation des Ellenbogens noch immer eine Herausforderung dar, und es gibt kaum einheitliche Behandlungsstrategien.
Methoden
Unterschieden werden einfache Luxationen ohne knöcherne Begleitverletzungen von den Luxationsfrakturen, auch komplexe Luxationen genannt. Für die knöchernen Verletzungen sind gute Klassifikationen erstellt worden, welche die Richtung der Therapie vorgeben. Im Fall der rein ligamentären einfachen Luxationen ist es bisher nicht gelungen, eine allgemein gültige Klassifikation zu erstellen, aus der sich ein bewährtes Behandlungskonzept ergibt. Welche Bandverletzungen konservativ ausheilen und welche in eine chronische Instabilität münden, ist genauso unklar wie die Vorhersagbarkeit einer möglicherweise eintretenden Gelenksteife. Die Herausforderung an den Kliniker besteht darin, die Verletzung bereits initial korrekt einzuschätzen und die richtige Therapie einzuleiten.
Schlussfolgerung
In diesem Artikel soll ein Behandlungspfad der einfachen ligamentären Ellenbogenluxation aufgezeigt werden, der in Abhängigkeit der durchgeführten differenzierten Diagnostik zu repräsentativ zuverlässigen Ergebnissen führt und die Fallstricke der Behandlung berücksichtigt.
Abstract
Background
Elbow dislocations are complex injuries which can lead to varying injury forms and degrees of severity. There are a variety of concepts for the treatment of elbow dislocations depending on the skill and experience of the treating physician. Elbow dislocations are still challenging injuries and there are nearly no standardized therapy concepts.
Methods
Dislocations can be divided into simple, if there are no accompanying bony injuries and fracture dislocations which are also called complex dislocations. There are quite good classifications for fractures which provide management guidelines. A grading system for simple dislocations with ligamentous damage has yet not been found and therefore problems of choosing the right treatment can occur. Ligamentous injuries which show good outcome with nonoperative treatment are hard to differentiate from those which lead to chronic instability and/or stiffness. The challenge is to initially judge the injury correctly in order to start the appropriate treatment.
Conclusions
This article presents guidelines for treating simple elbow dislocations according to the diagnostic findings which lead to reliable results and also considers the pitfalls.
