Zusammenfassung
Hintergrund
Die Humeruskopffraktur stellt mit einer Prävalenz von 4–5% eine der häufigsten Frakturen des Menschen, v. a. im höheren Lebensalter dar. Die primäre Hemiarthroplastik nimmt neben der konservativen Behandlung oder osteosynthetischen Verfahren immer noch einen geringen Stellenwert bei der Versorgung der Humeruskopfmehrfragmentfraktur ein. Ziel dieser prospektiven Studie bestand in der Evaluation der mittelfristigen Ergebnisse bei der Behandlung der dislozierten Humeruskopfmehrfragmentfraktur mit einer modularen Schulterendoprothese (EPOCA, Fa. Argomedical, Gifhorn).
Material und Methode
Von August 2000 bis Dezember 2002 wurden 24 Patienten, die sich eine dislozierte Humeruskopfmehrfragmentfraktur (IV/4, V/4, VI/4 nach Neer und „head split fracture“) zugezogen hatten, mit einer modularen Humeruskopfprothese versorgt. Das Durchschnittsalter der Patienten betrug 75,6 (52–92) Jahre. Alle Patienten wurden nach einem halben und nach einem Jahr zu einer klinischen und radiologischen Nachuntersuchung einbestellt.
Ergebnisse
Zu beiden Nachuntersuchungsterminen konnten 18 Patienten gesehen werden. Bei der Untersuchung wurde der Score nach Constant u. Murley (maximal 100 Punkte) und das UCLA-Rating-System (maximal 35 Punkte) zur Beurteilung der Funktion der Rotatorenmanschette erhoben. Ein Jahr nach der Operation waren 15 Patienten (83%) schmerzfrei. Der Score nach Constant u. Murley verbesserte sich von 52 (±17) auf 56 (±18) bei der 2. Nachuntersuchung nach einem Jahr. Auf der unverletzten Gegenseite wurde ein Score von 86 (±10) ermittelt (p<0,05 vs. operierte Gegenseite). Ebenso verbesserte sich die Funktion der Schulter gemessen am UCLA-Rating-System [von 25 (±4) auf 27 (±5) Punkte].
Schlussfolgerung
Der osteoporotische Knochen des alten Menschen erlaubt oftmals keine übungsstabile osteosynthetische Versorgung einer Humeruskopfmehrfragmentfraktur. In dieser Situation ist die primäre Hemiarthroplastik das Verfahren der Wahl.
Abstract
Background
Proximal humerus fractures are one of the most common fractures in elderly patients. In the treatment of complex proximal humerus fractures, primary hemiarthroplasty is still discussed controversially. The present study was undertaken to evaluate the results of primary hemiarthroplasty in the treatment of proximal humerus fractures with a modular prosthesis (EPOCA, Fa. Argomedical, Gifhorn, Germany).
Material and methods
A prospective study of 24 patients with complex humerus fracture (NEER: IV/4, V/4, VI/4, and head split fracture) was performed from August 2000 to December 2002. Mean age was 75.6 years (range: 52–92); 18 patients were seen for follow-up after 0.5 and 1 year, respectively. The Constant-Murley score (max. 100 points) and the UCLA Rating System (max. 35 points) were calculated for functional assessment of the operated shoulder.
Results
Fifteen patients (83%) were pain free 1 year after the operation. The Constant-Murley score improved from 52 (±17) to 56 (±18) at the second follow-up after 1 year. On the contralateral side a score of 86 (±10) was assessed (p<0.05 vs operated side). Correspondingly, shoulder function according to the UCLA Rating System improved [25 (±4); 27 (±5)].
Conclusions
Osteoporotic bone of older patients often may not permit stable internal fixation of complex proximal humerus fractures. In these situations primary hemiarthroplasty is the treatment of choice.
