Zusammenfassung
Die Behandlung psychotischer Erkrankungen ist aufgrund der Heterogenität in Ursächlichkeit und Verlauf, therapieresistenter Symptome, mangelnder Therapieadhärenz und hoher Rückfallraten eine Herausforderung für Therapeuten und Institutionen. Diesen Umständen kann durch eine Flexibilisierung therapeutischer Ansätze und Settings wirksam begegnet werden. Ein sinnvolles, aber bisher selten angewandtes Therapie- und Organisationskonzept ist die Behandlung psychotischer Störungen im Rahmen der sog. „Track-Einheit“. Eine „Track-Einheit“ ist als eine syndromal orientierte, dezentral arbeitende, modular aufgebaute, stationär-teilstationär-ambulante Einheit definiert. Das Track-Konzept bietet ein einheitliches, flexibel stadienadaptiertes Behandlungssetting in allen Stadien einer psychotischen Störung sowie die gesicherte Behandlungskontinuität. Ein weiteres Ziel ist es, möglichst viele Betroffene über das ganze Spektrum psychotischer Störungen rechtzeitig für eine Behandlung zu erreichen, um krisenhafte Zuspitzungen der Symptomatik abzuwehren und die Symptome bestmöglich zu adressieren und die Lebensqualität der Betroffenen nachhaltig zu verbessern oder zu bewahren. Das Track-Konzept soll aber nicht nur die aktuelle Versorgungssituation von Patienten mit psychotischen Störungen verbessern, sondern es versteht sich als eine dringend notwendige Investition in die Zukunft. Mit diesem Behandlungsmodell wollen wir sicherstellen, dass die guten, aber komplexen und aufwendigen Therapiemöglichkeiten auch dann Patienten zur Verfügung stehen, wenn krankheitsmotiviert keine Behandlung auf einer Station erwünscht ist.
Abstract
The treatment of psychotic disorders and illnesses is a challenge for therapists and institutions due to the heterogeneity of the cause and course, refractory symptoms, lack of therapy adherence and high rates of relapse. These circumstances can be effectively counteracted by the flexibility of therapeutic approaches and settings. A useful but rarely used concept is the treatment of psychoses within the so-called track unit. A track unit is defined as a syndrome-oriented, decentralized, modular unit, adjusted to the patient’s individual stage-specific needs across both inpatient and outpatient sectors. The track concept offers a fully integrated sector-spanning model of treatment at all stages of psychotic illnesses as well as a continuity of treatment. Another important goal is the early availability of timely treatment for as many psychotic patients as possible so that the symptoms can be alleviated as soon as possible and the quality of life can be sustainably improved or preserved. The track concept not only improves the current situation of treatment for acutely or chronically psychotic patients but also represents a necessary investment in the future. This treatment model aims to ensure that the good but complex and costly treatment options are available to patients even if inpatient treatment is not favored by the patient.

Der Gedanke der maximalen Kontinuität lehnt sich an den ersten Wirkfaktor der Psychotherapie nach Klaus Grawe an, nämlich die „Qualität der therapeutischen Beziehung“.
Zur Früherkennung und Behandlung psychischer Erkrankungen (z. B. Psychosrisikosyndrom) beim Übergang vom Jugend- in das Erwachsenenalter kann ein Spezialangebot im Sinne eines Adoleszentenzentrums für Psychose und Suchterkrankung sinnvoll sein.
Die stationsäquivalente Behandlung wird von Teammitgliedern der Track-Einheit SP-A gewährleistet und umfasst Ärzte, Psychologen, Pflegekräfte, Ergo- und Physiotherapeuten sowie Sozialarbeiter.
Beim SI- und Balancetraining handelt es sich um eine multiprofessionell geleitete Gruppe, mit der Aufgabe sowohl die sensorische Integration (v. a. taktile, vestibuläre und propriozeptive Funktionen) als auch die posturale Kontrolle des Patienten zu verbessern. Inhalte sind Einzel‑, Partner- und Gruppenübungen zur Verarbeitung von Sinneseindrücken und ein gezieltes Gleichgewichtstraining. Ziele sind eine verbesserte Wahrnehmung sowie die Reduktion neurologischer Softsigns (NSS) mit Störungen im Bereich komplexer sensomotorischer und kognitiver Leistungen bei Schizophrenie.
