Zusammenfassung

Emotionale Faktoren wie Depressionen und Angststörungen sowie somatoforme Störungen stellen ernst zu nehmende Prädiktoren für eine schlechte Prognose der Tinnitusentwicklung dar, die im Gesundheitswesen oft zu wenig beachtet werden. Eine mögliche Folge ist das Unterschätzen der Beeinträchtigung und Belastung des Tinnituspatienten und eine möglicherweise falsche oder zu späte Weichenstellung im Rahmen der Therapie. In der S3-AWMF-Leitlinie „Tinnitus“ ist der Thematik psychischer Störung daher großer Raum gegeben worden. Damit gelingt der therapeutisch evident wichtige Brückenschlag zwischen somatischer und psychischer Sichtweise für die Therapie von chronischen Tinnituspatienten. Bereits in der Anamneseerhebung bei Patienten mit einem dekompensierten Tinnitus sollte daher ein besonderes Augenmerk auf mögliche komorbide psychische Erkrankungen gelegt werden.

Abstract

Health authorities underestimate the negative influence of emotional factors such as depression, anxiety disorders, or somatoform disorders in the development of tinnitus and their contribution to a poor prognosis. This can lead to underestimation of the impairment suffered by tinnitus patients and, consequently, a possibly incorrect treatment approach or belated initiation of therapy. Mental disorders play a large role in the S3AWMF “Tinnitus” guidelines. Thus, the somatic and psychiatric approaches for treatment of chronic tinnitus patients are combined. Starting with taking chronic tinnitus patients’ case history, special attention should be paid to comorbid mental disorders.
