Operationsziel
Wiederherstellung der Funktion des vorderen Kreuzbandes (VKB).
Indikation
Chronische funktionelle Instabilität mit VKB-Ruptur mit subjektiven Instabilitätsgefühlen, akute VKB-Ruptur mit begleitender Meniskusrefixation.
Kontraindikationen
Lokale Infektion im Bereich des Kniegelenks, lokale Weichteilschädigung, Insertionstendinopathien des Streckapparats, Z. n. Quadrizepssehnenruptur, mangelnde Kooperationsbereitschaft des Patienten.
Operationstechnik
Entnahme des mittleren Sehnendrittels unter Schonung der gelenknahen Sehnenfasern mit einem Knochenblock über einen 4–5 cm langen Hautschnitt ausgehend vom oberen Patellapol. Bohrung des femoralen Kanals (Bohrung 0,5–1 mm kleiner als der Durchmesser des entnommenen Knochenblocks) für das Kreuzbandtransplantat über ein tiefes anteromediales Portal bei mehr als 110° gebeugtem Knie. Schonende Tunnelpräparation durch Einsatz von Dilatatoren. An der Tibia dient das Außenmeniskusvorderhorn bei fehlendem VKB-Stumpf als Landmarke. Die tibiale Bohrung sollte 0,5 mm größer als der Knochenblockdurchmesser sein, um einen problemlosen Transplantateinzug nach intraartikulär zu ermöglichen. Bei Revisionen werden Transplantatreste und Implantate aus dem Tunnel entfernt. Die femorale Fixation erfolgt implantatfrei, indem der Knochenblock im femoralen Tunnel verklemmt. Tibiale Fixierung des Transplantats mit resorbierbarer Interferenzschraube und Fixationsknopf.
Weiterbehandlung
Während der Entzündungsphase (1.–2. Woche) Schmerz- und Ergussprophylaxe (20-kg-Teilbelastung). Während der Proliferationsphase (2.–6. Woche) Belastung und Beweglichkeit langsam steigern. Während der Remodellierungsphase (ab 6. Woche) Kraftaufbau und Koordinationsübungen. In Revisionsfällen und in Abhängigkeit von Begleitverletzungen ggf. verlängerte Phase der Teilbelastung. Rückkehr zum Wettkampfsport ggf. verspätet, im Regelfall nicht vor dem 6.–8. Monat.
Ergebnisse
In einer prospektiven Studie wurden 33 Patienten (Alter 16–48 Jahre) nach VKB-Ersatzplastik mit der Quadrizepssehne nach 2 Jahren untersucht (12 Revisionen, 21 Primäroperationen). Keine post- oder perioperative Komplikationen. Postoperative Röntgenkontrolle mit anatomischer Tunnellage und ohne Dislokation des Knochenblocks. Die nach 2 Jahren mit dem KT 1000 gemessene Differenz der anterioren Translation der Tibia zum Femur besserte sich, verglichen mit der Gegenseite, von durchschnittlich 7,2 auf 2,2 mm in der Revisions- und von 6,4 auf 1,7 mm in der Primäroperationsgruppe. Nachweis eines gleitenden Pivot-shift-Phänomens bei 2 Patienten in der Revisions- und 1 Patienten in der Primäroperationsgruppe.
Abstract
Objective
Restore function of the anterior cruciate ligament (ACL).
Indications
Chronic functional instability with rupture of the ACL, giving way phenomena, acute rupture of the ACL with concomitant meniscus repair, rerupture of ACL graft with anatomical tunnels.
Contraindications
Local infection of the skin at the knee joint, local soft tissue damage, after rupture of the quadriceps tendon, enthesopathia of the quadriceps tendon, lack of patient compliance.
Surgical technique
Harvest quadriceps tendon graft with a bone block via a 4–5 cm long incision, starting from the middle third of the proximal patella pole without damaging the tendon fibers. Drill the femoral tunnel via a deep anteromedial portal with the knee flexed of more than 110° (tunnel diameter 0.5–1 mm smaller in diameter than bone block). Gentle tunnel preparation using dilators. In absence of an ACL stump the lateral meniscus anterior horn serves as tibial landmark. In case of revision surgery, remove graft material and implants from the tunnel. Graft fixation using press fit method in the femoral tunnel. Tibial graft fixation archieved with a resorbable interference screw and a button.
Postoperative management
Goal of the inflammatory phase (weeks 1–2) is pain and inflammation control (20 kg partial weight bearing). During the proliferative phase (weeks 2–6), load and mobility slowly increased (closed-chain exercises). During the remodeling phase (> 6 weeks), strength and coordination exercises are performed. In revision cases and in case of concomitant injuries, longer partial weight-bearing period might be necessary. Athletes should not return to competitive sports before 6–8 months.
Results
In a prospective study, 33 patients (age 16–48 years) were examined after replacement of the ACL with a quadriceps tendon graft after a minimum follow-up (FU) of 2 years (12 revision; 21 primary surgery). No post- or perioperative complications. Postoperative radiographs showed an anatomical tunnel location and no dislocation of the bone block. After 2 years the difference of a-p translation compared to the other leg was assessed by the use of KT 1000. The revision group improved from an average of 7.2 mm (pre-op) to 2.2 mm (FU). The group with primary surgery improved from 6.4 mm (pre-op) to 1.7 mm (FU). A sliding pivot shift phenomenon was detected in 2 patients in the revision group and 1 patient in the primary surgery group.
