Summary
The origin of the carcinoma lobulare in situ (clis) is traced to a proliferation of the basal myothelia (myoepithelial cells) in terminal and small excretory ducts of large segments. The clis grows through transformation and by incorporating normal cells at its periphery. This kind of growth is different from the growth through uncontrolled multiplication of a new race of cells characteristic of invasive carcinoma. Thus the clis behaves more like a hyperplasia or dysplasia. As it may provide the soil for the development of an infiltrating carcinoma, it plays the role of a precancerous lesion rather than that of a real carcinoma. The origin and details of development of an infiltrating carcinoma from a clis however are not as well established as for the so-called carcinoms in situ of other organs and therefore need further investigation.
Zusammenfassung
Das Carcinoma lobulare in situ (Clis) der Mamma wird auf eine mit einer gewissen Entdifferenzierung einhergehenden Wucherung der basalen Myothelien in Drüsengängen und terminalen Ausführungsgängen zurückgeführt. Sie befällt flächenhaft größere Abschnitte der Drüsengänge und der Ausführungsgänge und breitet sich aus durch Einbeziehung immer neuer Normalzellen in die Veränderung. Schon dadurch unterscheidet sich das Clis von einem aus sich heraus wuchernden Carcinom und verhält sich viel eher wie eine Hyperplasie oder Dysplasie. Da sich auf diesem Boden ein infiltrierendes Carcinom entwickeln kann, wäre das Clis viel eher zu den Präcancerosen als zu den Carcinomen zu rechnen. Allerdings ist die Krebsentstehung auf diesem Boden und ihr Ablauf im einzelnen dabei keineswegs so sichergestellt wie beim sog. Carcinoma in situ anderer Lokalisationen.
