Summary
42 patients with clinical signs of “brain death” or total brain infarction were examined by cerebral angiography—either unilateral carotid angiography (22 cases) or aortocranial angiography (20 cases), combined in 19 by selective angiography of one or more neck vessels. The intracranial circulation was found to be arrested in all but one patient, in whom a “global luxury perfusion” was present. It is contended that this latter finding may represent an early stage in the development of a total brain infarction. Aortocranial angiography by direct injection into the aorta is considered the method of choice for estiblishing the diagnosis of “brain death” (total brain infarction).
Résumé
42 patients présentant des signes cliniques de “mort cérébrale» ou d'infarctus total du cerveau ont été examinés par angiographie cérébrale, soit par angiograpie carotidienne unilatérale (22 cas), soit par angiographie aotocrânienne (20 cas) avec en plus chez 19 patients une angiographie sélective d'un ou de plusieurs vaisseaux du cou. La circulation intracrânienne était arrêtée chez tous les malades, sauf un, chez qui existait une «global luxury perfusion”. On peut ademettre que cette dernière découverte représente une phase initiale dans l'installation d'un infarctus total du cerveau. L'angiographie aortocrânienne par injection directe dans l'aorte est considérée comme méthode de choix pour l'etablissement du diagnostic de «mort cérébrale” (infarctus total du cerveau).
Zusammenfassung
Angiographische Untersuchungen von 42 Patienten mit der klinischen Diagnose eines Hirntodes. Die ausführlichen Untersuchungen der Autoren zeigten, daß die aortocraniale Angiographie mittels direkter Injektion des Kontrastmittels in die Aorta als die Methode der Wahl angesehen werden muß, um die Diagnose eines Hirntodes zu stellen.
