Summary
In 11 patients with lichen planus the cellular infiltrate was examined with the electron microscope. In 3 cases for a better differentiation of cell types ultrastructural-cytochemical reactions (acid phosphatase, peroxidase) were performed. The following results were obtained:

The infiltrate of Lichen planus consists predominantly of lymphocytes, a part of them in stimulated form. In addition, a remarkable number of monocytes, macrophages and transitional forms between these cell types can be found. They can be differentiated from lymphocytes by their ultrastructure as well as cytochemically.

A number of plasma cells, mast cells, dermal Langerhans cells, neutrophiic and sometimes also eosinophilic leucocytes can be seen regularly in the Lichen planus infiltrate. “Lutner cells” could also be observed.

Monocytes and macrophages sometimes exhibit hemidesmosome-like plaques, intercellular contacts were rarely present.

The cellular composition of the Lichen planus infiltrate resembles that seen in delayed hypersensitivity reactions. On the basis of the presently known findings it may be assumed that the infiltrate in Lichen planus represents the immunological response to an epidermal allergen.
Zusammenfassung
Bei 11 L.r.pl.-Patienten wurde das celluläre Infiltrat elektronenmikroskopisch untersucht, in 3 Fallen wurden zur besseren Zelldifferenzierung noch zusätzlich ultrastrukturell-cytochemisehe Reaktionen (saure Phosphatase, Peroxidase) durchgeführt. Foldende Befunde konnten erhoben werden:

Das Infiltrat des L.r.pl. besteht vorwiegend aus Lymphocyten, ein Teil dieser Zellen liegt in stimulierter Form vor. Daneben finden sich zahlreiche Monocyten und Gewebsmakrophagen bzw. Ubergangsformen zwischen diesen beiden Zelltypen. Sie lassen sich sowohl auf Grund der Ultrastruktur als auch cytochemisch von den Lymphocyten abgrenzen.

Regelmäßig konnten im L.r.pl.-Infiltrat einzelne Plasma- und Mastzellen, dermale Langerhans-Zellen, neutrophile und gelegentlich auch eosinophile Leukocyten nachgewiesen werden. Ebenso wurden Zellen, wie sie Lutzner u. Jordan (1968) beim Sézary-Syndrom beschrieben, beobachtet.

Monocyten und Gewebsmakrophagen weisen gelegentlich halbdesmosomenartige Membranveränderungen auf, vereinzelt konnten auch intercelluläre Kontaktstellen nachgewiesen werden.

Die celluläre Zusammensetzung des L.r.pl.-Infiltrates deutet auf eine immunologische Reaktion vom Spättyp hin. Auf Grund der bisher bekannten Untersuchungsergebnisse wäre es denkbar, daß das Infiltrat die celluläre Immunantwort auf ein epidermales Allergen darstellt.
