Summary
The electron microscopic examination of 11 patients with Lichen planus (L. pl.) exhibited distinct pathological changes in the dermo-epidermal junction area. The presence of fibrin deposits in this region and the ultrastructure of colloid bodies already have been described in previous papers. The following results were obtained:

At the border of the L. pl. papule a separation of basal lamina (BL) from the plasma membrane of basal keratinocytes can be seen. In the center of the lesion multiplication, disruption and regeneration of BL is ovservable. The anchoring fibrils remain intact. These changes of the BL are interpreted as being caused by chronic irritation of basal cells.

In the stratum basale and lower stratum spinosum dyskeratotic (type A) and completely keratinized (type B) cells are formed. Sometimes they are connected with the adjacent keratinocytes by desmosomes. The ultrastructure of type B cells is identical with that of colloid bodies; it is therefore assumed that in L. pl. these structures are formed by “dropping” of keratinized basal cells. In addition swollen cells with a marked reduction of tonofilaments can be found.

The electron microscopic findings above described support the assumption that the formation of L. pl. papule starts in the basal region of epidermis.
Zusammenfassung
Die elektronenmikroskopische Untersuchung von 11 Lichen ruber planus-Fällen ergab massive pathologische Veränderungen im Bereiche der Epidermis-Cutis-Grenze. Über das Vorliegen von Fibrinablagerungen in dieser Region und die Ultrastruktur der sog. hyalinen bzw. kolloiden Körperchen wurde bereits in anderen Publikationen berichtet.
Folgende Befunde wurden erhoben:

Die Basallamina (BL) läßt vorwiegend in der Randzone der Efflorescenzen eine Abhebung vom Plasmalemm der basalen Keratinocyten erkennen. In den zentraleren Abschnitten finden sich Duplikaturen bzw. netzartige Verbreiterung, völlige Ruptur und Regeneration der BL. Die in die BL einstrahlenden Mikrofibrillen bleiben dabei weitgehend intakt. Diese Veränderungen werden als Folge einer auf die Basalzellschicht einwirkenden Noxe aufgefaßt.

Im Stratum basale und unteren Stratum spinosum lassen sich teils dyskeratotische (Typ A), teils vollständig keratinisierte (Typ B) Zellen nachweisen, die zum Teil noch durch Desmosomen mit der Umgebung verbunden sind. Die Ultrastruktur der als Typ B beschriebenen Elemente entspricht weitgehend jener der hyalinen bzw. kolloiden Körperchen. Es wird daher angenommen, daß die sog. “colloid bodies” beim L.r.pl. durch eine Abtropfung bereits in der Basalzellschichte verhornter Zellen entstehen. Daneben finden sich ödematös geschwollene Keratinocyten, deren Tonofilamentgehalt deutlich reduziert ist.

Die beschriebenen elektronenmikroskopischen Befunde unterstützen die Annahme, daß die Entwicklung der L.r.pl.-Efflorescenz ihren Ausgang in den basalen Epidermisschichten nimmt.
