Das Pankreasvenenblut von Hunden wurde operativ unter Umgehung der Leber direkt in die Vena cava abgeleitet. Durch

doppelte porto-cavale Transposition.
Eine porto-cavale Transposition führte regelmäßig zu einer schweren diffusen großtropfigen Verfettung der Leberzellen. Durch die zusätzliche Anlage einer aorto-cavalen Fistel zur Erhöhung des O2-Druckes und der O2-Sättigung des in die Leber fließenden Cavablutes und des Druckes in der Vena cava, womit das Cavablut dem Pfortaderblut ähnlich wurde, ließ sich die Leberzellverfettung regelmäßig wesentlich verringern.
Die Nüchternblutzuckerwerte veränderten sich nach Ableitung des Pankreasvenenblutes von der Leber nicht. Bei der intravenösen Glucose-belastung kam es dagegen zu einem signifikant erhöhten initialen Blutzuckeranstieg. Der Glucoseassimilationskoeffizient (K) nahm aber nur bei den Tieren mit portocavalem Shunt ab, wahrscheinlich als Folge der verminderten Leberdurchblutung. Es wird angenommen, daß die direkte Leberpassage des Insulins lediglich für die Verhütung von abnormen Blutzuckerspitzen bei Glucosebelastung von Bedeutung ist.
Der Blutzuckerabfall nach intravenöser Gabe von Tolbutamid und Insulin unterschied sich nach Ableitung des Pankreasvenenblutes von der Leber nicht signifikant von den präoperativen Werten.

Venous blood from the pancreas of dogs was led directly into the caval vein by operation detouring the liver. By:

Duplicate porto-caval transpositions.
Porto-caval transposition regularly causes severe diffuse fatty liver with large fat droplets. By additionally setting-up an aorto-caval fistula in order to increase the O2-pressure and the O2-saturation of caval blood flowing to the liver and the pressure in the caval vein, thus caval blood was made similar to portal blood, the fatty liver cell degeneration could be regularly reversed.
The fasting blood sugar values did not change upon leading venous pancreas blood around the liver. However, on intravenous glucose loading there was a significantly enhanced initial blood sugar increase. But only the animals with a porto-caval shunt showed a decrease of the glucose-assimilation coefficient (K). This was probably the result of the reduced hepatic blood flow. It is assumed that the direct liver passage of insulin is only significant in preventing abnormal blood sugar peaks on glucose loading. The blood sugar decrease after intravenous application of tolbutamide and insulin did not significantly differ from the pre-operative values.
