Summary
Intravenous injection of small amounts of an oily contrast material (Lipiodol UF) results in diffuse oil embolism in canine lung. Following oil-embolism the arterial oxygen tension decreases. 0,1–0,2 ml Lipiodol/kg body weight causes only a slight and temporary depression of arterial pO2 (−3,1 Torr), while with larger amounts (0,3 ml Lipiodol/kg body weight) a more severe and longer lasting depression of arterial pO2 (more than 20 Torr) occurs.
Infusion of Lipiodol is followed by a marked pulmonary hypertension and a mild fall of systemic blood pressure.
If the contrast material is uniformly spread all over the lung in small embolies there is greater disturbance than with larger but regionally limited embolic states.
The importance of these results for complications following lymphography in man is discussed.
Zusammenfassung
Die intravenöse Injektion kleiner Mengen eines öligen Kontrastmittels (Lipiodol UF) führt beim Kaninchen zu multiplen ölembolien der Lunge.
Diese Ölembolien beeinträchtigen den Gasaustausch in der Lunge im Sinne einer Diffusionsstörung und bedingen eine dosisabhängige Erniedrigung des arteriellen Sauerstoffdrucks. Während Lipiodol UF in Dosen von 0,1–0,2 ml/kg KG nur zu einem geringen und kurzdauernden Abfall des arteriellen pO2 führt (−3,1 Torr), kommt es bei der nur gering höheren Dosis von 0,3 ml/kg KG zu einer schwerwiegenden und mehrere Tage anhaltenden arteriellen Hypoxie (pO2-Abfall um mehr als 20 Torr). Als Ausdruck der Rechtsüberlastung steigt der Druck im rechten Ventrikel unter gleichzeitigem Abfall des Drucks im großen Kreislauf an. Bei diffuser feintropflger Verteilung des Kontrastmittels in den Lungen sind die Veränderungen ausgeprägter als bei größeren, aber regional begrenzten Ölembolien.
Die Bedeutung dieser Befunde für die lymphographiebedingten Lungenkomplikationen des Menschen wird diskutiert.
