Liver resections in cirrhotic patients: A western experience

Abstract

Systematic screening for hepatocellular carcinoma (HCC) in cirrhotic patients by alpha-fetoprotein and ultrasound permits the detection of small asymptomatic tumors. Owing to the small tumor size, more liver resections can now be performed than in the past. These resections are performed in a more economical way in terms of loss of functional parenchyma and in a more appropriate manner with regard to carcinology: surgical techniques of liver segmentectomy and use of intraoperative echography are mandatory. Favorable long-term results obtained in Eastern countries by resecting such lesions are encouraging but remain to be confirmed in the Western hemisphere.

Résumé

La recherche systématique d'un carcinome hépatocellulaire du foie chez les cirrhotiques par le dosage de l'alphafeto-protéine et l'exploration par ultrasons permet de découvrir des tumeurs asymptomatiques habituellement petites. En raison de la taille peu importante de la tumeur un plus grand nombre de résections hépatiques peuvent Être pratiquées plus souvent. Ces résections moins étendues limitent au minimum le sacrifice du parenchyme sain alors mÊme qu'elles sont plus efficaces sur le plan carcinologique: la réalisation de ces segmentectomies impose l'emploi de l'échographie opératoire. Les résultats favorables à long terme obtenus dans les pays orientaux sont encourageants mais demandent à Être confirmés dans les pays occidentaux.

Resumen

El tamizaje sistemático en busca de carcinoma hapatocelular en pacientes cirróticos mediante la determinación de alphafeto-proteína y el ultrasonido permite descubrir tumores asintomáticos, generalmente pequeños. En virtud del tamaño reducido del tumor, hoy se pueden realizar más resecciones hepáticas que en el pasado. Tales resecciones pueden ser ejecutadas en una forma más económica en términos de la pérdida de parenquima funcional, y en una forma mas apropiada en términos de los propósitos carcinológicos: las técnicas quirÚrgicas de segmentación hepática y el uso de ecografía intraoperatoria son mandatorios. Los resultados favorables logrados en los países orientales con la resección de tales lesiones son estimulantes pero aÚn deben ser confirmados en maestros países occidentales.
