Summary

The syndrome of inappropriate ADH secretion („SIADH“) was first recognized 1935 by Roth et al. and described in detail 1957 by Schwartz et al. The clinical symptoms (hyponatremia, hypertonicity of urine and inability to excrete a water load) are caused by inadequately elevated ADH secretion under a variety of situations and diseases.

Some recent work was focused on the pathogenesis of this syndrome and new clinical findings (low plasma levels of uric acid and potassium) as well as special forms („SIADH“ without elevated vasopressin levels in plasma) are thought to be of relevance. New therapeutical recommendations will be discussed.

Zusammenfassung

Die Kombination einer niedrigen Plasma-Natriumkonzentration mit einer inadäquat erhöhten Urin-Natriumausscheidung wurde erstmals 1935 von Roth und die dabei ursächliche Bedeutung einer erhöhten und nicht regulierten ADH-Sekretion dann ausführlich 1957 von Schwartz und Bartter beschrieben. Durch die Möglichkeit der ADH-Analyse im Plasma ist in letzter Zeit bei einer Vielzahl von pathologischen Situationen und zunehmend häufig das Syndrom der inadäquaten ADH-Sekretion nachgewiesen worden. Neue klinische Beobachtungen (Hypourikämie, Hypokaliämie), die Analyse anderer Hormonsysteme beim SIADH, Sonderformen („SIADH“ ohne erhöhten Plasma ADH-Spiegel) und neue therapeutische Empfehlungen lassen eine kurze Übersicht der heutigen Kenntnisse dieses Syndroms sinnvoll erscheinen.
