Summary
At present the diagnosis of cerebral death is based on the absence of manifestations of cerebral function (deep coma, wide pupils, absence of reflexes and spontaneous respiration, isoelectric EEG, etc.) during a variable period of time. However, the absence of vital signs of an organ, even for prolonged periods, is no proof of its death. The death of an organ can only be considered as proved by the demonstration that this organ has remained completely deprived of its blood flow during a period of time that surpasses its ability to survive circulatory arrest. In the case of the brain, the permanent absence of clearance of a radioactive tracer deposed within the brain is considered to demonstrate total absence of flow through it and, thus, to prove cerebral death. As discussed in the text, previous attempts at angiographic “demonstration” of the arrest of cerebral blood flow cannot be accepted uncritically.
The value of the present findings in determining the death of a patient with reference to organ transplantation has been discussed.

Zur Zeit stützt sich die Diagnose des zerebralen Todes auf das Fehlen von Zeichen zerebraler Funktion (tiefes Koma, weite Pupillen, Fehlen von Reflexen und Spontanatmung, isoelektrisches EEG usw.) während eines verschieden langen Zeitabschnittes. Indessen ist das Fehlen von Lebenszeichen eines Organes, selbst über längere Zeit, kein Beweis für dessen Tod. Der Tod eines Organes kann nur dann als gesichert gelten, wenn nachgewiesen ist, daß dieses Organ für eine Zeitspanne vollständig seiner Blutversorgung beraubt war, die seine Fähigkeit übersehreitet, einen zirkulatorischen Stillstand zu überleben. Im Falle des Gehirns wird die Ansicht vertreten, daß ein dauerhaftes Fehlen einer Clearance eines radioaktiven Indikators, der in das Hirn eingebracht worden ist, das vollständige Fehlen einer Durchblutung und damit den Hirntod beweist. Wie im Text diskutiert, können frühere Versuche des angiographischen Beweises eines Stillstandes der Hirndurchblutung nicht unkritisch akzeptiert werden.
Die Bewertung der dargestellten Befunde bezüglich der Feststellung des Todes eines Patienten in Hinblick auf Organtransplantationen wird diskutiert.
Résumé
Le diagnostic de mort du cerveau est basé jusqu' à présent sur l'absence de manifestations des fonctions cérébrales (coma profond, mydriase bilatérale, abolition des réflexes, arrêt respiratoire, tracés E.E.G. plats etc...) pendant un laps de temps variable. Cependant l'absence de signes fonctionnels d'un organe, même pendant un laps de temps prolongé, n'est pas la preuve formelle de sa mort. La mort d'organe ne peut être prouvée que par la démonstration que cet organe est resté complètement privé de circulation sanguine pendant un laps de temps qui dépasse sa capacité de survie à un arrêt circulatoire.
Dans le cas du cerveau, l'absence de clearance d'un produit radioactif déposé dans cet organe, est la preuve de l'arrêt total de la circulation sanguine et ainsi de la mort du cerveau. La démonstration de l'arrêt circulatoire par l'angiographie dans les travaux antérieurs, ne peut être acceptée sans réserves.
La valeur des découvertes résentes pour diagnostiquer la mort d'un patient, en cas de transplantation d'organe, a été discutée.

Al momento attuale la diagnosi di morte cerebrale si basa sull'assenza di manifestazioni della funzione cerebrale (coma profondo, dilatazione pupillare, assenza di riflessi e di respirazione spontanea, EEG isolelettrico ecc.) durante un periodo di tempo variabile. Tuttavia l'assenza di segni vitali da un organo anche per un prolungato periodo, non e' prova della sua morte.
La morte di un organo puo' essere considerata corne provata solo con la dimostrazione che questo organo e' rimasto privato della sua circolazione per un periodo tempo che sorpassa la sua capacita' di sopravvivere ad un arresto circolatorio.
Nel caso del cervello la assenza permanente della clearance di un marcante radioattivo depositato dentro il cervello e' considerata come dimostrazione dell'assenza totale della circolazione in esso e quindi come prova di morte cerebrale.
Come discusso nel testo, precedenti tentativi di una «dimostrazione» angiografica dell'arresto della circolazione cerebrale non puo' essere accettata senza critiche.
Il significato delle presenti ricerche per la determinazione della morte di un paziente e' discusso anche in rapporto ai trapianti di organo.

Actualmente el diagnóstico de muerte cerebral se funda en la falta de se¯nales de función cerebral (coma profundo, pupilas dilatadas, falta de reflejos y respiralión espontánea, EEG isoeléctrico, etc.) durante un espacio de tiempo de distinta duración. La falta de signos de vida de un organo, aún durante largo tiempo, no es una prueba de su muerte. La muerte de un órgano solo puede asegurarse cuando se ha demostrado que para ese órgano su irrigación de sangre fué suprimida totalmente durante un espacio de tiempo, el cual sobrepasa su capacidad de sobrevivir un paro cireulatorio. En caso del cerebro se defiende la opinión, de que la falta duradera de aclaramiento (Clearance) de un indicador radioactivo, transportado al cerebro, demuestra la falta completa de circulacion y con ello la muerte cerebral. Como se discute en el texto, no pueden ser aceptados sin crítica, pruebas anteriores de la demostración angiográfica de un paro de la circulatión cerebral.
Se discute la valoración de los hallazgos en relación a la demostración de muerte de un paeiente, en vista de una transplantación de órganos.
