Infantile subacute necrotizing encephalomyelopathy

Summary

A case of an uncommon neuropathological disorder in a 3 years 11 months old girl is reported. The parient presented a gradually progressive course manifested by an initial feeding difficulty and subsequent failure of normal growth and development, neurological signs and symptoms, and eventual death.

The postmortem study showed characteristic symmetrical lesions involving the walls of the third ventricle, thalami, subthalamic nuclei, red nuclei, substantia nigra, corpora quadrigemina, dentate nuclei of the cerebellum, floor of the fourth ventricle, olivary nuclei and adjacent gray matter of the medulla oblongata, and upper cervical spinal cord.

The histological characteristics of the lesions were typical of infantile subacute necrotizing encephalomyelopathy. The most severe fulminating changes with fresh hemorrhages superimposed on a rather old process were present in the gray matter of the medulla oblongata.

Destruction of myelin of the nerve fibers was observed in the optic tracts, lateral columns of the upper thoracic spinal cord and posterior nerve roots of the cervical and thoracic spinal cord.

Zusammenfassung

Bericht über eine ungewöhnliche neuropathologische Veränderung bei einem 3 Jahre 11 Monate alten Mädchen. Die Krankheit manifestierte sich durch eine anfängliche Ernährungsschwierigkeit und nahm einen allmählich progredienten Verlauf mit Störungen des normalen Wachstums und der Entwicklung, Auftreten neurologischer Ausfälle und schließlich Tod.

Die Autopsie zeigte symmetrische Läsionen an den Wänden des 3. Ventrikels, des Thalamus, des Nucleus subthalamicus, des roten Kernes, der Subst. nigra, der corpora quadrigemina, der Zahnkerne des Kleinhirns, am Boden des 4. Ventrikels, der unteren Oliven, der grauen Substanz der Medulla oblongata und des oberen Halsmarkes. Die histologischen Veränderungen sind typisch für die infantile subakute nekrotisierende Encephalomyelopathie. Die schwersten Veränderungen mit frischen Hämorrhagien überlagern ziemlich alte Läsionen in der grauen Substanz der Medulla oblongata. In den Tractus optici besteht Entmarkung der Nervenfasern, ebenso in den Seitensträngen des oberen Brustmarkes und in den hinteren Wurzeln des Hals- und Brustmarkes.
