Summary

A 4 years old boy died from subacute progressive panencephalitis after 5 months of illness. The clinical, morphological and histochemical findings of this case are demonstrated and discussed. Histologically, the characteristic picture of SPPE was found with infiltrations of plasma cells and lymphocytes, inclusion bodies of type A ofCowdry in oligodendrocytes, severe changes and destruction of ganglion cells and an intensive reaction of the micro- and astroglia. Histochemical studies showed a considerable increase of protein and RNA of the inclusion bodies and in the cytoplasm of the affected nerve cells, especially in the region of the brainstem. The significance of these results is discussed in relation to a possible virus etiology.

Zusammenfassung

Klinische, morphologische und histochemische Befunde eines typischen Falles von subakuter progressiver Panencephalitis (SPPE) werden demonstriert und diskutiert. Die Erkrankung betraf einen vierjährigen Knaben, der nach fünfmonatiger Leidensdauer verstarb. Histologisch fand sich das charakteristische Bild einer SPPE mit dichten lymphocytär-plasmacellulären Infiltraten, Einschlußkörperbefall der Oligodendroglia, ausgedehnten Nervenzellveränderungen und-untergängen und einer intensiven Reaktion der Mikro- und Astroglia. Histochemische Untersuchungen ergaben einen starken Protein- und RNS-Reichtum der Einschlußkörper und eine beträchtliche Vermehrung der Proteine und Ribonucleoproteide im Cytoplasma zahlreicher Nervenzellen, vor allem in den Kerngebieten des Hirnstammes. Die Bedeutung dieser Veränderungen im Hinblick auf eine Virusätiologie (Myxoviren) im Sinne einer „slow virus infection” wird diskutiert.
