Gentamicin was administered to 8 and 16 healthy pregnant women during labour by continuous intravenous automatic infusion of 20 mg/h (“low dose group”) and 40 mg/h (“high dose group”) respectively. Maternal serum and amniotic fluid concentrations were repeatedly determined during labour and maternal and cord serum concentrations after delivery.

In the maternal serum a steady state between 2.3–3 mcg/ml was obtained in the low dose group and between 4–5 mcg/ml in the high dose group.

Depending on individual maternal levels cord serum concentrations reached 1–2 mcg/ml in the low dose group and between 1 and 4.9 mcg/ml in the high dose group.

Amniotic fluid concentrations rose slower than those in the cord serum, but exceeded maternal serum concentrations after about 7 hours in the high dosage group. Thereafter the distribution ratios were between 1.2 and 1.48.

The intrauterine levels especially in the higher dose group were high enough to inhibit gentamicin-sensitive organisms after a lag time of 4–6 hours.

Gentamicin wurde 8 bzw. 16 gesunden Schwangeren mit gesprungener oder eröffneter Fruchtblase während der Geburt als intravenöse Dauerinfusion mit einer automatischen Infusionspumpe und einer Dosis von 20 mg/Stunde (niedrige Dosis), bzw. 40 mg/Stunde (höhere Dosis) als Infektionsprophylaxe infudiert. Mütterliche Serum- und Fruchtwasserkonzentrationen des Antibiotikums wurden wiederholt während der Geburt und zusammen mit der Konzentration im Nabelschnurblut nach der Entbindung bestimmt.

Das steady state im mütterlichen Serum lag bei der niedrigen Dosierung zwischen 2,3–3 mcg/ml und bei der höheren Dosierung zwischen 4–5 mcg/ml.

In Abhängigkeit von individuellen mütterlichen Serumspiegeln erreichten die Nabelschnurserumkonzentrationen 1–2 mcg/ml bei der niedrigeren Dosierung und 1–4,9 mcg/ml bei der höheren Dosierung.

Die Fruchtwasserkonzentrationen stiegen langsamer an als die Nabelschnurserumkonzentrationen; sie überschritten jedoch die mütterlichen Serumkonzentrationen nach etwa 7 Stunden bei der höheren Dosierung. Danach lag der Verteilungsquotient zwischen 1,2–1,48.

Die intrauterinen Spiegel waren besonders bei der höheren Dosierung hoch genug, um gentamicinempfindliche Keime nach einem Intervall von 4–6 Stunden zu hemmen oder abzutöten.
