Summary
Experimental subjects, fastened by means of a parachute harness to a tilt-table, were subjected to passive orthostasis. With assumption of the vertical position, the venous tone manifested a very marked initial constrictive reaction. With continued orthostasis, the values for venous tone remained regularly above those recorded in the horizontal position. While heart rate as a rule increased during the experiments, a considerable decrease in pulse was observed in most cases, with the onset of syncopal symptoms. Whereas respiratory rate increased, tidal volume demonstrated relatively small and variable changes.
With the subjects standing in a tank, active orthostasis was effected by modification of gravitational effect on the lower half of the body resulting from hydrostatic pressure alterations. With reduction of gravity effect, the venous tone decreased in direct relation to the water level, and attained, after discontinuance of filling, a thereafter constant value. With decrease then of the water level, venous tone increased and, in the majority of cases, surpassed the values observed in the control period. In contrast, the peripheral venous pressure behaved in the opposite manner, increasing with a rising water level, and decreasing with a falling level. The significance of venous tone adjustments for position-dependent circulatory regulation, and the responsible mechanisms thereof, are discussed.
Zusammenfassung
Bei passiver Orthostase waren die Versuchspersonen mit einem Fallschirmgurt auf einem Kipptisch befestigt. Nach Übergang in die vertikale Position zeigte der Venentonus sehr starke initiale konstriktorische Reaktionen. Im weiteren Verlauf lagen die Werte bis zum Ende der passiven Orthostase regelmäßig über den Werten bei horizontaler Position. Während der passiven Orthostase nahm die Herzfrequenz zu, fiel jedoch beim Auftreten von Kollapssymptomen teilweise erheblich wieder ab. Die Atemfrequenz stieg an, das Atemzugvolumen zeigte relativ geringe und uneinheitliche Veränderungen.
Bei aktiver Orthostase standen die Versuchspersonen aufrecht in einem Behälter, die Gravitationseffekte auf die unteren Körperabschnitte wurden dabei durch hydrostatische Druckwirkungen modifiziert. Bei Reduzierung der Gravitationseffekte nahm der Venentonus in Abhängigkeit vom Wasserspiegel kontinuierlich ab und erreichte nach Abschluß der Behälterfüllung ein weitgehend konstant bleibendes Niveau. Bei zunehmenden Gravitationseffekten stieg der Venentonus mit absinkendem Wasserspiegel wieder an und überschritt in der Mehrzahl der Versuche die Werte der Vorperiode. Der periphere Venendruck zeigte demgegenüber ein weitgehend spiegelbildliches Verhalten mit Zunahmen bei ansteigendem und Abnahmen bei absinkendem Wasserspiegel.
Die Bedeutung der Venentonusumstellungen für die Kreislaufregulation bei Lagewechsel sowie die an ihrer Auslösung beteiligten Mechanismen werden diskutiert.
