Summary
The influence of a short respiratory acidosis on standard bicarbonate and some other blood parameters was investigated in 10 humans breathing a gas mixture of 7.5% CO2 in O2 for 15 min. Before and after the period of hypercapnia the subjects respired pure oxygen. pH,pCO2, standard bicarbonate, plasma protein-and haemoglobin concentrations, haematocrit and electrolyte concentrations were determined in nearly arterialized venous blood and plasma. The plasma values were measured after anaerobic sampling of blood and after equilibration of blood with nearly 40 mm HgpCO2 at 38°C (conditions of standard bicarbonate).
During the respiratory acidosis venous pH dropped from 7.40 to 7.28. Standard bicarbonate concentration decreased about 1 mval/l compared with the preceding period of oxygen breathing. The haemoglobin content of the blood increased about 0.4g-%, the haematocrit about 1.5%. The concentrations of electrolytes in whole blood remained almost unchanged. In plasma water the sodium concentration increased about 5 mval/l while in erythrocyte water the sodium concentration decreased 6 mval/l and the chloride concentration increased 4 mval/l. After equilibration with nearly 40 mmHgpCO2 Na+ions shifted bock to the cells and Cl− ions back to the plasma. Thus the excess of alkali ions in the plasma was diminished and less bicarbonate could be formed. The main cause of the electrolyte shifts is probably changed activity of the ion pumps in the cell membranes.
Zusammenfassung
Um den Einfluß einer akuten respiratorischen Acidose auf das Standardbicarbonat und einige Blutparameter zu untersuchen, atmeten zehn Versuchspersonen bis zu 15 min lang ein Gasgemisch aus 7,5% CO2 in O2. Vorher und nachher erhielten sie reinen Sauerstoff. pH,pCO2, Standardbicarbonat, Plasmaeiweiß- und Hämoglobinkonzentration, Hämatokrit sowie die Elektrolytkonzentrationen wurden in weitgehend arterialisiertem Venenblut und im Plasma gemessen. Die Plasmawerte wurden sowohl unmittelbar nach anaerober Blutentnahme wie auch nach Äquilibrierung des Blutes mit ca. 40 mm HgpCO2 bei 38°C bestimmt (Standardbicarbonatbedingungen).
Während der respiratorischen Acidose sank das venöse pH von etwa 7,40 auf 7,28 ab. Das Standardbicarbonat verringerte sich gegenüber der vorangehenden O2-Atmungsperiode um 1 mval/l. Die Hämoglobinkonzentration stieg um 0,4 g-%, der Hämatokrit um 1,5% an.
Die Elektrolytkonzentrationen im Vollblut blieben bei Hyperkapnie fast unverändert. Im Plasmawasser stieg dagegen die Natriumkonzentration um etwa 5 mval/l an, während im Erythrocytenwasser die Natriumkonzentration um 6 mval/l ab- und die Chlorkonzentration um 4 mval/l zunahm. Wurde jetzt das Blut unter Standard-bicarbonatbedingungen äquilibriert, wanderte Natrium zurück in die Erythrocyten und Chlor zurück in das Plasma, so daß der Kationenüberschuß im Plasma abnahm und damit weniger Kohlensäure in Form von Bicarbonat gebunden werden konnte. Als Ursache der Elektrolytverschiebungen spielt anscheinend eine veränderte Tätigkeit der Ionenpumpen eine wichtige Rolle.
