Abstract
Derived from comprehensive clinical and blood gas analytical studies performed on 479 patients treated in the intensive care unit for CO-intoxication since 1957, a classification of subacute CO-intoxication into three stages of severity is made on the basis of level of consciousness as well as respiratory, circulatory and acid-base status.
In the absence of a relationship between the degree of severity of the intoxication and HbCO-concentration in the blood at the time of hospital admission, stages II and III are characterized by a metabolic acidosis resulting from previous CO-caused tissue hypoxia and by signs of progressive circulatory shock. Immediate treatment of this condition by alkalization with NaHCO3 and mechanical ventilation with oxygen by concomitant muscle relaxation has allowed a reduction in mortality from 11.2 to 4.2 percent.
