Abstract
In 12 obese children the theoretical changes in the body composition during total starvation were studied. By this means a fat loss of about 44% was established even in the first few days of fasting. According to the energy requirements, 2 weeks of starvation amounted to a mean cumulative fat loss of 3.12 kg (235 g per day), i.e. about 48.2% of the observed weight loss was due to catabolism of body fat. Since the inevitable loss of fat-free body mass (about 26 g protein per day) was considerable, therapeutic fast in childhood should be reserved for grossly obese children found to be resistant to any other therapeutic management. Longer periods of total fasting such as are recommended for adults must be refused resolutely in obese children.
Zusammenfassung
Bei 12 adipösen Kindern wurden die theoretischen Veränderungen der Körperzusammensetzung während der absoluten Nahrungskarenz ermittelt. Danach kam es bereits in den ersten Fastentagen zu einem Nettofettverlust von ca. 44%. Der kumulative Fettverlust betrug entsprechend den energetischen Bedürfnissen der Patienten in einer zweiwöchigen Fastenperiode im Mittel 3,12 kg (235 g/die), d. h., etwa 48,2% der beobachteten Gewichtsabnahmen wurden durch einen Abbau von Körperfett verursacht. Da die unvermeidlichen Verluste an fettfreier Körpermasse (ca. 26 g Protein/Tag) nicht unerheblich sind, kann das thera-peutische Fasten im Kindesalter nur bei extremer Fettsucht empfohlen werden, wenn alle anderen Behandlungsversuche versagen. Längerdauerndes Fasten, wie es bei Erwachsenen zur Anwendung kommt, ist für adipöse Kinder strikt abzulehnen.
