Die paraartikulären Ossifikationen — eine Komplikation der Totalendoprothese des Hüftgelenkes

Ectopic ossification: A complication after total hip replacement

Summary

The incidence of ectopic ossification after total hip replacement so that their influence on pain, mobility, gait and prognosis are discussed following a review of 200 cases. The most likely main cause of ossification appears to be a constitutional predisposition. As a matter of fact it was above all encountered by young and strong men or sportmen. The surgical technique, especially the detachment of the great trochanter (or the muscle release) seems to be of importance. Other factors such as duration of operating, lengthening of femur, type of prosthesis and using acrylic cement, seem to be without influence.

Following this review, emphasis is laid on fine surgical technique, avoiding traumatism of periost or soft tissues, recommending carefull haemostasia and postoperative suction drain. By appearing of the first ossification, the rehabilitation should be more slowly and gentle conducted. If necessary the ossification should be operated out at soonest 1 or 1.5 years after its apparition.

Zusammenfassung

An Hand eines Krankengutes von 200 unausgelesenen Fällen von Totalendoprothesen des Hüftgelenkes wurde die Häufigkeit postoperativer paraartikulärer Ossifikationen, ihr Ausmaß und ihre Bedeutung für Störungen der operierten Patienten (Bewegungseinschränkungen, Schmerzen, Gang, Prognose) erörtert. Als Ergebnis der Untersuchung ist festzuhalten, daß als wahrscheinlichste und hauptsächlichste Entstehungsursache dieser Ossifikationen eine gewisse Disposition zu diesen Verkalkungen anzunehmen ist, da hauptsächlich kräftige, sportlich und körperlich geübte Männer von 40–50 Jahren davon betroffen werden. Auch die Art des operativen Vorgehens, besonders die Abmeißelung oder Abschälung am Trochanter major, erhöht zweifellos die Häufigkeit dieser Komplikationen. Andere Momente, wie Operationsdauer, verstärkte Muskelspannung durch Verlängerung der Extremität bei der Operation, Art der angewandten Implantate und Verwendung von Knochenzement haben nach dem mitgeteilten Krankengut keinen Einfluß auf die Entstehung dieser Ossifikationen. Als zwingende Schlußfolgerungen der Untersuchung wird gefordert, daß ein möglichst schonendes operatives Vorgehen angewandt wird, wobei ausgedehnte Periostablösungen und Gewebstraumatisierungen zu vermeiden sind. Hämatome sollen durch Blutstillung und Saugdrainagen vermieden bzw. entfernt werden. Beim ersten Auftreten dieser Ossifikationen dürfen nur noch vorsichtige aktive Bewegungsübungen durchgeführt werden, während alle sonstigen exogenen Reize zu vermeiden sind. Eine operative Beseitigung der paraartikulären Ossifikationen sollte frühestens 1 Jahr bis 1 1/2 Jahre nach der primären Operation vorgenommen werden und hat dann eine gewisse Aussicht auf bleibendem Erfolg.
