Zusammenfassung
Heterotope Ossifikationen nach Hüftgelenkersatz spielen heute weniger eine Rolle als dies vor 20 Jahren der Fall war. Wesentliche Eckpunkte zur Vermeidung solcher Ossifikationen sind das muskelschonende Operieren, das ausgiebige Spülen mit Entfernung sämtlicher Knochenpartikel sowie die Ossifikationsprophylaxe. Heterotope Ossifikationen finden sich im Wesentlichen bei muskelkräftigem Patienten oder bei Patienten, bei denen der Zugang zum Femur aus anderen Gründen erschwert war. Die insgesamt schwierigere Erreichbarkeit des Femurs bei vorderen Zugängen scheint somit auch eine Rolle für die Häufigkeit periartikulärer Ossifikationen zu spielen. Für die Entfernung von Ossifikationen sollte die Finalisierung der Verknöcherungen abgewartet werden, die im Regelfall nach 2 Jahren erreicht ist. Eine frühzeitige Revision führt hier zu einer hohen Rezidivwahrscheinlichkeit. Ist die Entfernung von Ossifikationen geplant, ist neben der medikamentösen Prophylaxe die prä- oder postoperative Bestrahlung unbedingt in die Therapieplanung mit einzubeziehen.
