Zusammenfassung

Bei Präexzitationssyndromen ist die normodrome (rechtläufige) Erregungssequenz von den Vorhöfen zu den Kammern über das AV-Knoten-His-Purkinje-System (Erregungsleitungssystem = ELS) oder auch die retrograde Erregungssequenz durch die Existenz zusätzlicher akzessorischer Erregungsleitungsbahnen verändert. Da die Erregung über die akzessorische Leitung keine physiologische Verzögerung wie im AV-Knoten erfährt, wird das Ventrikelmyokard in unterschiedlichem Ausmaß vorzeitig erregt. Diese Vorzeitigkeit der Erregung veranlaßte Öhnell (1944) zu dem Begriff Präexzitation, der sich mit dem von Holzmann (1965) gewählten Begriff Antesystolie inhaltlich deckt.
