Zusammenfassung
Mitochondriopathien zählen zu den häufigsten neurometabolischen Erkrankungen im Kindes- und Jugendalter. Sie sind klinisch äußerst heterogen, der Übergang ins Erwachsenenalter ist fließend. Betroffen sind vor allem energiebedürftige Organe. Genetische Defekte gibt es im mitochondrialen Genom und häufiger noch in der nukleären DNA. Biochemisch unterscheidet man Defekte der Atmungskettenenzyme, der mitochondrialen DNA-Replikation und Proteinsynthese, von Substraten, Kofaktoren und Homöostase der Organelle. Durch die Entwicklungen der Next Generation Sequenzierung steht die Genetik in der Diagnostik jetzt weit vorne, es gibt einen Shift zu „Genetics first“. Die Therapie ist nach wie vor hauptsächlich symptomatisch. Präimplantationsdiagnostik und Mitochondrien-Transfer sind wichtige neue Ansätze der Pränatalmedizin. Wichtig für die Abklärung, Therapie und Patientenführung ist eine enge Zusammenarbeit mit Kompetenzzentren, vor allem auch zur Entwicklung neuer Behandlungsoptionen.
