Abstract

Disease emerges as a “deviation from normal,” with symptoms and signs observed initially in isolated instances, then in groups of patients, reappearing in larger series in increasingly identifiable clinical patterns.1 In time, information accumulates not only on the clinical spectrum of the illness but also on its pathology, pathophysiology, and therapy. This sequence broadly defines the pattern of recognition of ulcerative colitis (UC) and Crohn’s disease (CD) many years earlier. Once regarded as medical curiosities, the “nonspecific” inflammatory bowel diseases today are among the more important of human illnesses, affecting millions of people worldwide.

Nature does not proceed by leaps, but rather reveals its secrets slowly, quietly and grudgingly. Notable advances of today have their background in work often carried out decades before
